Perinatal clinicians are confronted with the problem that they are supposed to gather information about the condition of the unborn child almost exclusively by monitoring the rate of the fetal heart. Human heart rate recording, however, is no more or less informative in utero than in the newborn nursery. Determination of the pre-ejection period (PEP) of the fetal heart might provide information regarding the performance of the heart of the human fetus in utero and about its cardiovascular adjustments during fetal distress. On the verge of introduction of clinical human fetal PEP monitoring, this article presents a review of the results obtained from chronically instrumented fetal animals as well as some preliminary data from human perinatal research. Finally, possible future developments in human fetal PEP recording are anticipated and it is concluded that the most promising feature of the PEP for clinical use appears to be its predictive value as an early antepartum indicator of possible fetal distress during subsequent labor.

摘要:
The presence of an enkephalin-like substance (‘humoral endorphin’) in human maternal and cord blood, as well as in amniotic fluid, has been demonstrated in this study. Utilizing antibodies to leucine-enkephalin, it was found that the level of this substance in maternal and in cord blood remained stable during pregnancy whereas the concentration in amniotic fluid was significantly higher at mid-trimester than during labor at term.

Uterine hyperactivity can be treated with uterine relaxants. In order to establish the most satisfactory dosage for Fenoterol, several doses were administered intravenously to pregnant women during birth and the actions on uterine relaxation and on side effects were examined. Saline was injected in a group of patients as control. With doses of 20 and 40 μg Fenoterol, submaximal uterine relaxation times of 7.3 and 7.9 min were obtained. Subjective side effects were observed, such as palpitations and restlessness; maternal blood pressure was unchanged. The pulse rate increased significantly only after 40 μg. 10 μg Fenoterol induced a uterine relaxation time of 5.6 min, was without side effects and was therefore regarded as the most satisfactory dosage. With 5 μg Fenoterol a relaxation time of only 3.2 min was observed. In the control group, administration of saline was without any observable effect.

摘要:
115 normotensive healthy primigravidae have been investigated during 28–32 weeks of pregnancy for early detection of pregnancy-induced hypertension (PIH) by performing the roll-over test (ROT) of according to Gantetal. No statistically significant relation was found between a positive supine pressor test and development of PIH. The hydrostatic effect, calculated in 15 of these women, seems to influence the results of the ROT. The other factors which can influence the blood pressure with change of position have also been enumerated.

摘要:
The pituitary endocrine function in 4 anencephalic infants was studied by measuring the concentrations of growth hormone, prolactin, thyroid-stimulating hormone (TSH), follicle-stimulating hormone (FSH) and luteinizing hormone (LH) in umbilical venous serum. There was no significant difference in hormone levels between the anencephalic and normal infants. The response of FSH and LH was negative after administration of 10 μg of luteinizing hormone-releasing hormone. The TSH levels after administration of 50 μg of TRH elevated about 4- to 6-fold over pre-administration levels. These results indicate that the hypothalamo-pituitary axis of the human fetus initiates its function from mid-pregnancy; TSH may have an autoregulating system of the pituitary itself.

The registry for trophoblastic disease in Wakayama, Japan was formed and 342 cases were analyzed. The total number of trophoblastic disease was 342 during 6 years (1972–1977). The number of hydatidiform moles, invasive moles, choriocarcinoma and transitional moles was 278 (81.3%), 28 (8.2%), 18 (5.3%) and 12 (3.5%), respectively. The greatest number of hydatidiform moles occurred in the range 25–29 years inclusive. The percentage of patients with hydatidiform moles over 45 years of age was significantly high. The incidence of malignant complications (invasive moles and choriocarcinoma) of hydatidiform moles was 16.5% in 284 molar pregnancies. The frequency of malignant complications was remarkably high in women over 40 years of age. Thus the epidemiologic study of trophoblastic disease might turn up some important data and the importance should be emphasized.
