Tubal microsurgery is a valuable tool to study tubal reproductive physiology and various infertility problems. Microsurgical tubal resection reanastomoses of the rabbit fallopian tube and their impact on fertility are reviewed. Data obtained from experimental work on the function of cilia and muscle activity in tubal transport are discussed.

Serum levels for β-human chorionic gonadotropin (β-hCG) and pregnancy-specific β1-glycoprotein (SP1) in patients with trophoblastic disease were measured by radioimmunoassay and enzyme-linked immunosorbent assay. The β-hCG:SP1 ratios were below 1.0 in all 22 cases of complete hydatidiform mole and in 8 of 9 cases of partial hydatidiform mole. Two (10.5%) of 19 cases of invasive mole involving metastasis had ratios that rose above 1.0 during chemotherapy. Ratios ranged from 1.6 to 29 in 11 of 15 cases of choriocarcinoma before chemotherapy. The remaining 4 cases, diagnosed within 3 months of antecedent pregnancy, had ratios below 0.99. Thus, the difference between choriocarcinoma and nonchoriocarcinoma β-hCG:SP1 ratios may be due to trophoblastic differentiation based on the developmental stage and with trophoblast age, or due to the mass and potential activity of trophoblastic cells.

We have previously shown a marked diurnal variation in the excretion of cortisol and 6β-hydroxycortisol at 36–38 weeks pregnancy with no variation in estrogen excretion. Since others have shown a reciprocal relationship between plasma levels of estriol and cortisol at 30–35 weeks of pregnancy, we decided to determine whether a similar relationship was present in urinary concentrations of cortisol, 6β-hydroxycortisol and estriol at this gestational age. A significant variation in excretion of both cortisol and 6β-hydroxycortisol was found. The variation in urinary levels of estriol was less than that of the corticosteroids but followed the same pattern.

Heart rate (HR), heart rate variability (HRV), respitatory rate (RR) and transcutaneous PO2 and CO2 of 12 infants born by elective cesarean section were monitored during the first postnatal hour in order to evaluate the effect of the β-mimetic tocolytic drug ritodrine. Six of the mothers received ritodrine by infusion and the other 6 physiological saline during the 2 h prepartum. The Apgar scores of the treatment group were higher (p < 0.05) but there was a significant increase in RR (p < 0.05) during the first hour in these babies. Arterial blood pressure (BP) was lower in the group whose mothers had received ritodrine (p < 0.05) but there was no difference in pulse pressure. Transcutaneous pressure of CO2 (PtcCO2) of the controls decreased during the first hour (p < 0.05). Transcutaneous pressure of O2 (PtcO2) increased during the first postnatal hour (p < 0.01) when both groups were examined together. HR of both groups was relatively high and HRV low.

Pregnant women with high estrogen levels were more sensitive to barbiturate-induced narcosis and hypothermia than nonpregnant and postmenopausal women. Within each group, there was a negative correlation between estrogen level and decreased sensitivy to barbiturates. It appears that estrogen alters resistance to barbiturates through its effect on the activity of dopamine receptors.

We measured the concentration of circulating prolactin (PRL) in the serum during ovulatory and anovulatory cycles of normoprolactinemic menstruating women. The results demonstrate that during the luteal phase of ovulatory cycles a significant increase of PRL is observed together with that of progesterone. On the contrary, in menstruating women with anovulatory cycles we could not detect these increases. These data, analyzed together with the concomitant changes of gonadotrophins secretion, suggest a possible luteotrophic action of PRL in humans during the menstrual cycle.

Two aromatase inhibitors, 4-hydroxyandrostenedione (4-OHA) and testololactone (Teslac), were tested to determine their effects on folliculogenesis, particularly ovarian histologic alterations, in the cycling rat. Adult, female Sprague-Dawley rats were treated with continuous infusion of both inhibitors at concentrations of 10––8, 10––4, and 10––2M for 30 days. The effect of the inhibitors on cultured granulosa cells harvested on proestrus was determined in vitro. The in vivo administration of each inhibitor induced significant reduction in ovarian-vein estradiol levels. Estradiol synthesis in cultured granulosa cells was inhibited by both aromatase inhibitors in a dose-dependent fashion. These observations indicate that 4-OHA and Teslac significantly inhibit basal estradiol synthesis in vivo and in vitro. This effect on estrogen synthesis was not reflected in alteration of the ovarian histology in the cycling rat.

The efficacy of low-dose oral contraceptives containing 30 μg or 35 μg ethinyl estradiol in combination with Levonorgestrel, gestoden (Δ15-Levonorgestrel) and cyproterone acetate was studied. Borderline doses for inhibition of ovulation were found to be approximately 50 μg for Levonorgestrel, 40 μg for gestoden and 1 mg for cyproterone acetate. These results were derived from the assay of luteinizing hormone, follicle-stimulating hormone, 17β-estradiol and progesterone serum levels during the daily treatment of a total of 47 female volunteers from day 5 through day 25 of their cycles. A combination of 30 μg ethinyl estradiol plus 75 μg Levonorgestrel or 75 μg gestoden resulted in complete inhibition of ovulation in each of the 20 subjects studied. Monitoring of hormone serum levels in 6 female volunteers with normal cycles during the administration of 35 μg ethinyl estradiol plus 2 mg cyproterone acetate showed inhibition of ovulation in all subjects studied during the first and third therapy cycle. One subject exhibited follicular maturation during the third treatment cycle as judged from the patterns of 17β-estradiol serum levels. Present data suggest that low-dose oral contraceptives containing Levonorgestrel, gestoden and cyproterone acetate are highly efficient in providing great contraceptive safety and do not support the previous notion that the kind of progestagen used is relevant for the degree of central inhibition.

The activities of six enzymes involved in energy metabolism were measured in leiomyoma specimens and in the adjacent normal myometrium from the uterus of 17 patients. In leiomyomas the specific activities of hexokinase, lactate dehydrogenase and hydroxyacyl-CoA dehydrogenase (HAD) were higher than in myometrium. The soluble protein content was lower in leiomyomas. Therefore, most of the differences of specific activities were not found when the enzyme activity was expressed per gram wet weight of tissue, except for HAD activity, which was still higher in leiomyomas (p < 0.05). This result is compatible with increased fatty acid utilization by leiomyomas, and with the increased growth potential of such tumors.

The serum levels of CA 125 and CA 19-9 were determined by an immunoradiometric assay employing the monoclonal antibody OC 125 and anti-CA 19-9 antibody in 88 patients with ovarian carcinoma. When a cut-off value of CA 125 was set below 35 U/ml in the control group, serum elevated levels of CA 125 were found in 86.7% of the patients with surgically demonstrable ovarian serous cystadenocarcinoma, in 100% (4/4 cases) of clear-cell carcinoma, in 50% (2/4 cases) of endometrioid carcinoma, in 100% (5/5 cases) of undifferentiated carcinoma, and in 80% of the recurrent cases. Using a cut-off value of 37 U/ml, serum elevated levels of CA 19-9 were detected in 68.2% of mucinous cystadenocarcinoma, in 28.9% of serous cystadenocarcinoma, in 75% (3/4 cases) of metastatic ovarian carcinoma, and in 37.5% of the recurrent cases. A statistical analysis of the combination assay using CA 125, CA 19–9, tissue polypeptide antigen (TPA), immunosuppressive acidic protein (IAP), ferritin and CEA was carried out by multivariate method (discriminatory analysis) in 45 patients with ovarian carcinoma and 50 healthy subjects. As a result before treatment, positive rates of a single tumor marker were 79.7% with CA 125, 42.7% with CA-19-9, 73.1% with IAP, 61.7% with TPA, 64.3% with ferritin and 25.4% with CEA, respectively. A combination assay of these markers was useful for detecting identification of ovarian carcinoma, by which it gave a higher accuracy of ovarian cancer detection.
