Echocardiographic hemodynamic and left ventricular parameters were determined in 14 normotensive and 18 hypertensive women during the last trimester of pregnancy. The hypertensive patients had significantly higher mean values of total peripheral resistance (p < 0.001), maximum velocity of posterior wall motion (p < 0.05), mean velocity of circumferential fiber shortening (p < 0.01) and percent fiber shortening of left ventricular diameter (p < 0.05). The mean values for heart rate, stroke volume, cardiac output, cardiac index, left ventricular posterior wall thickness and septal thickness did not differ significantly in both groups. These results reflect increased cardiac contractility related to elevated peripheral vascular resistance in subjects with pregnancy-induced hypertension.

In 345 women with a favorable cervical score at due date, labor was either induced by means of intravaginal application of tablets containing 3 mg of prostaglandin E2 or spontaneous onset of labor was awaited until the 42nd week of pregnancy. Eighty percent of the nulliparae and 96.3% of the multiparae of the induction group gave birth within 24 h of the administration of the first tablet. All pertinent delivery intervals were significantly shorter in this group compared to those women where spontaneous onset of labor was awaited. The rate of operative deliveries was lower in the induction group. With the exception of 1 fetal death 3 days after due date, fetal outcome was excellent in both groups. Elective induction was at least equivalent to awaiting the onset of spontaneous labor and was not associated with higher complication rates due to the method of induction.

The effects of ritodrine, administered by intravenous, intramuscular or per os routes, upon plasma levels of unconjugated estriol, estradiol, progesterone and human placental lactogen (HPL) were investigated in 24 third-trimester pregnancies. Ritodrine administered at a constant rate of 200 μg/min for 3 h, injected intramuscularly 8×10 mg/day for 5 days or given orally in doses of 6 × 20 mg/day for 10 days resulted in a significant decrease in the steroid hormone concentration and a significant elevation of HPL levels.

In a population of 1,050 pregnant women the effect of maternal colonization by group B Streptococcus on premature rupture of membranes (PROM), preterm delivery, and low weight was analyzed. A significant increment was found of the prevalence of PROM for patients colonized in the vagina and/or the rectum (26.4%) versus noncarrier patients (17.8%). In vaginal and/or rectal group B Streptococcus carriers, in whom group B Streptococcus was also isolated from the cervical culture, the rate of PROM was higher (41.7%), while when the cervical culture was negative, the PROM was similar to noncarriers. There were no significant differences with respect to colonization conditions regarding the incidence of preterm delivery or the different preterm delivery indicators analyzed.

Thyroxine-binding globulin (TBG) and free thyroxine (FT4) were studied in 172 apparently healthy women giving birth to malformed children. The malformations were divided into two groups: minor malformations (group A) and major malformations (group B). The blood samples were usually drawn in the first trimester. Group A showed no significant difference in TBG values compared to women with healthy children. Group B showed lower, but not significantly lower values in earlier pregnancy. However, from the 15th-16th week of pregnancy the TBG values were significantly lower. The FT4 values were in the hyperthyroid range in as much as 20 % of the women, compared to the reported rate of 0.2% hyperthyroidism in pregnant women.

A case-control study was carried out to investigate the association between maternal smoking during pregnancy and the risk of early neonatal jaundice. A total of 1,569 cases of neonatal jaundice (2.7% of all singleton births) were identified from the 1984 Washington State birth certificates as having a bilirubin level greater than lOmg/dl within the first 2 days of life. 2,336 nonjaundiced infants were randomly selected to serve as controls. Information regarding smoking during pregnancy was also obtained from the birth certificates. After excluding infants with known risk factors for neonatal jaundice, 912 cases and 1,752 controls were available for analyses. Infants whose mother smoked cigarettes during pregnancy were at lower risk of neonatal jaundice (odds ratio=0.81; 95% confidence interval=0.66–0.99) relative to infants of nonsmokers. This relative risk changed very little after adjusting separately for maternal age, infant’s gender, number of prior pregnancies, number of prenatal visits, marital status and number of prior fetal deaths. There was suggestive evidence that this apparent protective effect may be operating only among normal and high-birth-weight infants.

Tissue specimens from uterine cervix and corpus of non-pregnant and pregnant women were incubated in vitro in the presence of 3H-norepinephrine. The uptake in the cervix exceeded those of the lower and upper segments of the corpus, and this difference was most pronounced at term pregnancy. The neuronal uptake in the cervix constituted approximately 80% of the total uptake and was higher than in isthmus and fundus (50 and 20%, respectively). The results favour the notion that cervical nerves remain functionally intact throughout pregnancy and support previous histochemical studies demonstrating a segmental difference in uterine innervation and a partial denervation of the myometrium at term.

Since not all problems in endometriosis can be studied in the human, there is need of an animal model. We transplanted endometrial tissue of the rabbit into the peritoneal cavity and studied the changes after 12 weeks. At that time the endometrial implants in the rabbit had become cystic structures. The macroscopic and histologic appearance was consistent with endometriosis. Hormonal supplementation turned out not to be necessary. Microsurgical techniques prevented the formation of tubo-ovarian adhesions. This makes the rabbit model suitable for studying the influence of endometrial implants on fertility.
