The fetal heart rate (FHR) response to a vibroacoustic stimulation was studied in 100 women during active labor. Three types of FHR responses were noted: acceleration, tachycardia of duration longer than 2 min, and combined acceleration-deceleration response. The last type was significantly more common in labors with abnormal FHR tracings. All fetuses demonstrating FHR response to the stimulation, including those with abnormal FHR tracings, had good outcome. The vibroacoustic stimulation is easily performed, noninvasive, and has no contraindications. Its possible application as an intrapartum test of fetal well-being awaits further clinical confirmation.

Elevated plasma concentrations of β-lipotropin (β-LPH) and β-endorphin (β-EP) are present in vaginally delivered babies in the first 24 h of life. To establish if fetal distress or different types of delivery influence the secretion of these peptides in the neonatal period, we studied 16 vaginally delivered newborns (VD), 11 neonates extracted through elective cesarean section (CS) and 8 babies suffering from acute intralabor fetal distress (FD). In all groups of newborns the plasma levels of β-LPH and β-EP were measured in arterial and venous cord blood, at 1 and 24 h from delivery. β-LPH and β-EP were determined by specific radioim-munoassays after silicic acid extraction and Sephadex G-75 column chromatography. In cord plasma, there were no differences between arterial and venous concentrations of either β-LPH or β-EP in any of the groups. In the 1st h after delivery the plasma levels of VD newborns were similar to umbilical cord values (β-LPH: 19.9 ± 5.2; β-EP: 18.1 ± 2.4), while those of CS (β-LPH: 30.6 ± 4.5; β-EP: 30.1 ± 5.8) and FD (β-LPH: 64.9 ± 11.8; β-EP: 65.7 ± 24.4) showed a significant increase. At the 24th h of life the plasma concentrations of both peptides decreased significantly in the three groups, but CS and FD babies showed significantly higher plasma values than VD newborns (p < 0.001). These data indicate that neonatal opioid secretion is an event typical of birth, independent of the mode of parturition, whose significance could be an adaptive response to the new environment.

A liquid chromatographic technique for the detection of metronidazole (MDZ) and its major metabolites, combined with a vaginal sampling technique employing extraction of the drug from vaginal swabs, was used to evaluate the concentration of MDZ in the vaginal fluid during therapy. Wide variation in the absolute concentration of unchanged drug was noted, although the average vaginal levels were about half the serum concentration 2 h after treatment had been initiated and comparable to the serum levels 6 and 24 h after treatment. Oral administration of MDZ invariably resulted in the presence of the drug in the serum and urine but the drug was not always detected in the vaginal secretions. The presence of the relatively inactive acetyl metabolite in the vaginal fluid of patients who failed to respond to therapy for trichomoniasis suggested the possible role of drug modification as a contributing factor in the lack of therapeutic success. The presence of MDZ in women in whom the cervix had previously been surgically removed attests to the transport or transudation of the drug across the vaginal epithelium without involvement of the secretory activity of the cervix.

Bacteriological isolation of anaerobes, Gardnerella and lactobacilli was carried out in a group of 62 women with the diagnosis bacterial vaginosis and 42 control women. Lactobacilli were the predominant organisms in the control group whereas anaerobes dominated the flora in bacterial vaginosis patients. Lactate-gel (pH 3.5, 5 ml) inserted into the vagina daily for 7 days is as effective as oral metronidazole, 500 mg twice daily for 7 days. The women in both groups became symptom-free and objectively improved. Anaerobes were significantly reduced (p < 0.0001) in both groups after 1-week treatment but Gardnerella was not significantly reduced. As bacterial vaginosis is generally looked upon as a mild noninflammatory condition lactate-gel seems to be an ideal treatment for this disease.

All 3 triploid conceptuses examined had a common, dispermic origin. When histological diagnoses were reconsidered in this light 1 conceptus turned out to be nonmolar. This supports recent case reports which indicate that dispermic triploids may be either molar or nonmolar.

The behavioural pattern of transplantable mouse fibrosarcoma (MFS) cells infused into the pregnant, pseudopregnant and nonpregnant mouse uteri was studied histologically. In some nonpregnant and pseudopregnant uteri, the tumour cells, without colonizing inside the uterus, traverse the endometrium and within 5 days of treatment form tumour nodules outside the myometrium, indicating the presence of an initial and temporary tumour rejection mechanism in these uteri. In later stages MFS cells form large necrotic tumours in these uteri. In the pregnant uterus no tumour nodules are formed outside the myometrium even after 20 days of treatment. Furthermore, the size of the tumour formed in the endometrium of the pregnant uterus is quite small and the tumour cell growth rate is lower than that of the tumour cells in the nonpregnant and pseudopregnant uteri. These results indicate that, in the pregnant mouse uterus, fibrosarcoma cells display an altered pattern of invasion and decreased growth rate compared to those in nonpregnant and pseudopregnant uteri.

The natural killer (NK) and antibody-dependent cellular cytotoxic (ADCC) activities were determined in peripheral blood lymphocytes of patients with gynecologic malignancy. The effects of a preparation of Streptococcuspyogenes (OK-432) and of interferon were determined. Patients with advanced cancer exhibited lower natural cytotoxicity than normal women. NK and ADCC activities were decreased following surgery and NK activity was decreased following chemotherapy. Radiation reduced the percentage of Leu-7-positive cells in the circulation of the patients. OK-432 augmented the NK activity of both healthy women and patients with early malignancy. Interferon was less effective than OK-432 in augmenting NK activity.
