摘要:
The basic architecture of colonic electrolyte transport has been well described. Recent research, using sophisticated electrophysiologic techniques such as patch-clamping, have permitted investigators to characterize the properties of ion-specific channel proteins, that is, to characterize their molecular physiology. Further efforts have been directed at exploring the intricate web of interrelated factors—immune cells, enteric neurons, and paracrine cells—that contribute to the fine regulation of epithelial transport.

There has undoubtedly been progress in our management of both primary and advanced or recurrent colorectal cancer. Surgery remains the mainstay and most important modality. However, adjuvant therapy for patients thought to have had a “curative” resection may be beneficial. Promising approaches include systemic treatment involving 5-fluorouracil in conjunction with levamisole and portal-vein infusion in conjunction with 5-fluorouracil. In addition, radiotherapy has been shown to reduce local recurrence rates. For advanced or recurrent disease, several novel approaches are beginning to show promise, including enhancement of the effect of 5-fluorouracil by biochemical modulation with, for example, leucovorin. In addition, the combination of immunotherapy with chemotherapy may result in an enhanced response. These approaches deserve further study.

This review addresses important contributions to the literature over the past year in two main areas: surgical management of colorectal cancer (with perioperative care) and inflammatory bowel disease (IBD), with some discussion of outcome following IBD surgery. Because in so brief a communication the reader is not able to fully savor each of the articles mentioned, I attempt to flavor this review with my impressions.

This is a review of selected articles from the literature over the past year and a half on a variety of colonic disorders. Case reports of neutropenic colitis,Balantidium coliinfestation, amebic colitis, tuberculous colotis, andListeriainfection are included. Eleven reports of collagenous and microscopic colitis are summarized. Two studies dealing with colonic concentrations of short-chain fatty acids are discussed. Other areas covered include brown bowel syndrome, abdominal epilepsy, asymptomatic ileocolitis, and diabetic diarrhea.

The previous year's publication in diagnostic imaging attempted to clarify the relative value of the various modalities in a number of clinical settings. The role of the barium enema in the management of colorectal neoplasms and diverticulitis continues to be questioned by proponents of primary colonoscopy with respect to the former and computed tomography with respect to the latter. Multiple promising reports have supported the potential value of transrectal ultrasound for staging rectal cancer and detecting its recurrence. Similarly impressive data concerning the accuracy of abdominal ultrasound with graded compression in suspected appendicitis have been presented. Whether such success proves to be universal remains to be determined. Conversely, after earlier enthusiastic reports on the benefits of defecography in selecting patients for correction of defecation disturbances, recent papers are somewhat more reserved, particularly with regard to certain diagnostic parameters previously believed to be significant. Thus, the pendulum continues and will continue to swing in the approach to these diagnostic problems.
