Objective: Investigation of the bacteriological quality of processed wound blood in orthopedic hip surgery depending on antibiotic prophylaxis or not. Design: Prospective randomized study after approval of the Ethics Committee. Setting: Operation theatre/intensive care unit of an Orthopedic University Clinic. Patients: 40 patients underwent first hip arthroplasty under regional anesthesia. 20 received as antibiotic prophylaxis cefuroxime 1.5 g single shot, 20 not. Interventions: Preopera-tive collection of autologous plasma (2 × 900 ml), normovolemic hemodilution (15 ml/KG), intra- and postoperative mechanical autotransfusion (Cell Saver®/Autotrans®). A quantitative bacteriological study was performed from the collecting bag (Vacufix®), wound drainage blood and processed red blood cell concentrates (PRBCC), using a pour plate technique and broth culture enrichment (Bactec® bottles). Examination after 2 and 7 days. Results: Maximally 1 CFU/ml (colony-forming unit) of bacteria was isolated from 8 Vacufix® bags of the control group (8/20), in contrast to no growth in the antibiotic group (0/20; p < 0.005). Wound drainage blood was found sterile in all patients. In 8 PRBCC of the control group (8/20) bacteria were identified in a concentration of < l (5×), 1-2 (2×), and 130 (l×) CFU/ml. In the cefuroxime group only 3/20 showed bacterial growth < l CFU/ml. Conclusions: Bacterial contamination of processed wound blood never caused wound or systemic infections. Ziel: Vergleich einer standardisierten Ernährungslösung mit einer selbst-gemischten Infusionstherapie bei Intensivpatienten einer chirurgischen Intensivstation; Untersuchung der standardisierten Ernährungstherapie auf Praktikabilität und Effizienz. Design: Prospektive randomisierte Studie. Rahmen: Chirurgisch-traumatologische Intensivstation einer Universitätsklinik. Patienten: Chirurgisch-traumatologische Patienten, die länger als 6 Tage auf der Intensivstation lagen. Interventionen: Gabe einer standardisierten Ernährungslösung im Ein-Liter-System (ZPE-GE) bzw. Gabe einer herkömmlichen, selbstgemischten Infusionslösung (hER). Bestim-mung von Stoffwechselparametern und Serum- bzw. Harnelektrolyten, zusätzlich Bestimmung von Blut- und Harnglukose, Harnstoffproduktionsrate, Triglyzeriden. Ergebnisse: Die beiden Patientengruppen waren sowohl in ihren physiologischen Daten als auch in der Schwere der Erkrankung miteinander vergleichbar, Komplikationen und Unverträglichkeiten konnten unter den angegebenen Regimen nicht beobachtet werden. Die Energie- (durchschnittlich 1202 kcal/Tag in der Gruppe hER versus 1331 kcal/Tag in der Gruppe ZPE-GE) und Substratzufuhren (Glukose: 228 versus 252 g/Tag; Aminosäuren: 56 versus 64 g/Tag) sind als ausge-glichen zu betrachten, es konnten keine Stoffwechselentgleisungen beobachtet werden. Die Elektrolytzufuhr (Natrium: 56 mmol/Tag in der Gruppe hER versus 92 mmol/Tag in der Gruppe ZPE-GE; Kalium: 65 versus 68 mmol/Tag; Phosphat: 15 versus 19 mmol/Tag) scheint in der Vollperiode nicht ganz optimal zu sein. Schlussfolgerungen: Die standardisierte Lösung scheint gegenüber den herkömmlichen Mischungen Vorteile zu haben: Sie erleichtert das Handling im Intensiv- und Postoperativbereich und verbessert Genauigkeit, Zeitersparnis und hygienische Bedingungen beim Umgang mit diesen Lösungen. Eine Infusionsmenge von 2000 ml (300 g Glukose und 100 g Aminosäuren) dieser Lösung erscheint als ausreichend, bei höherer Zufuhr könnte die Kohlenhydrat- und Aminosäurenzufuhr zu hoch liegen. Auch bei der Elektrolytzufuhr ist eine Begrenzung auf 2000 ml (120 mmol Natrium) angezeigt. Der Natriumanteil könnte auf 50 mmol/l reduziert werden, um ein entsprechendes Natriumloading zu verhindern, bei der Phosphatzufuhr ware eine Erhöhung auf 12 mmol/l empfehlenswert. Die Lösung kann zur standardisierten parenteralen Ernährung bei einem Großteil (ca. 80%) der postoperativen und posttraumatischen Patienten herangezogen werden, nur bei Extremsituationen müssen weiterhin die Einzelkomponenten (Glukose, Aminosäuren, Fette und Elektrolyte) speziell zusammengemischt werden, um metabolische

Objective: Our experimental studies have indicated that the initial distribution volume of glucose (IDVG) has potential to evaluate the central extracellular fluid (ECF) volume status in the body. The purpose of the present study was to compare the IDVG and cardiac output (CO) in critically ill patients with or without expansion of the central ECF volume and to test whether a larger IDVG is observed during the presence of increased central ECF volume than during the absence of it. Design and Setting: The study was prospective and was conducted in the intensive care unit, University of Hirosaki Hospital, after approval of the institutional ethical committee. Patients: Fourteen patients with accumulation of the central ECF volume (the accumulation group) and 29 patients without it (the non-accumulation group) were studied. Interventions: Glucose 5 g was infused over 30 s when the patients’ hemodynamic states were relatively stable. Blood samples were taken immediately before infusion and during 7 min post-infusion. Thermodilution CO was also measured before each glucose challenge. A total of 81 comparisons were performed. Measurements and Results: The IDVG was calculated using a one-compartment model. A linear correlation was obtained between the IDVG and CO in the non-accumulation group (r = 0.89, n = 54, p < 0.001). The IDVG-CO ratio was higher in the accumulation group than in the non-accumulation group (p < 0.01). The increased plasma glucose concentrations at 3 min postinfusion were inversely proportional to the IDVG (r = -0.90, n = 81, p < 0.001). Conclusions: Results demonstrated that a larger IDVG was obtained during accumulation of the central ECF volume and support the hypothesis that the IDVG reflects the central ECF status in critically ill patients characterized in our study, even though CO influences the IDV

Background: Peripheral blood progenitor cells (PBPC) can be collected by most blood cell separators. The present study was designed to compare two new techniques for PBPC collection. Design: Prospective, randomized study. Setting: Haemapheresis Unit and Haematological Division of a University Clinic. Patients: 20 patients with non-Hodgkin’s lymphoma, Hodgkin’s disease and thyroid cancer. Material and Methods: 20 patients with malignancies underwent PBPC collection with a ‘reduced-volume’ programme (Fresenius AS 104) and a single-needle technique (Haemone-tics MCS 3p), respectively. PBPC were mobilized by combined chemotherapy and cytokine administration or application of growth factor alone. Blood counts, CD 34 expressing cells and mononuclear cells (MNC) were determined before apheresis and in the PBPC product. Granulocyte macrophage colony-forming units (CFU-GM) were determined in the apheresis product. PBPC were frozen at controlled rates and stored in liquid nitrogen until they were used for autologous transplantation after high-dose chemotherapy. Results: 32 PBPC collections were carried out with the Fresenius AS 104 and 27 separations with the Haemonetics MCS 3p device. Median product volume was 106 ml using the Fresenius AS 104 apparatus and 156 ml in the case of the Haemonetics MCS 3p machine. The median CD 34-expressing cell count was 1.0×106/kg body weight for the Fresenius device and 0.6 ×106 for the Haemonetics cell separator. The cell recoveries of the two studied devices were significantly different. Six patients were grafted after high-dose chemotherapy. Five showed signs of haematopoietic recovery after autologous PBPC retrans-fusion. Conclusions: The study reported here revealed that PBPC could be collected by both techniques. Although the collected amounts of CD34 expressing cells were similar, cell recoveries were significantly higher in the Haemonetics machine. Using the ‘reduced-volume’ procedure no additional centrifugation prior to cryopreservation was necessary to remove the plasma from the apheresis product. Hintergrund: Ziel der Untersuchung ist die Ermittlung der Prävalenz von HTLV-I/II bei Dauerblutspendern in Deutschland zur Beurteilung eines möglichen Übertragungsrisikos durch Bluttransfusionen. Material und Methodik: 43185 Serum-proben von 11303 Dauerblutspendern des Jahres 1993 wurden auf HTLV-I/II-Antikörper untersucht (Abbott, HTLV-I-EIA). Als zusätzlicher Test für alle wiederholt positiven Proben diente ein ELISA (Pasteur, Platelia® HTLV-I New), als Bestätigungstests wurden ein Immunoblot [Diag. Biotechnology (Pte) Ltd., HTLV Blot 2.3] und die PCR (Hoffmann-La Roche, Amplicor® HTLV-I/II) verwendet. Ergebnisse: Die im HTLV-I-EIA reaktiven Spender wurden in vier Gruppen unter-teilt: positiv, grenzwertig, potentiell serokonvertiert und alternierend. Mit Hilfe von Bestätigungstests konnte jedoch nur bei einer Spenderin eindeutig eine persistierende HTLV-II-Infektion nachgewiesen werden. Eine HTLV-I-Infektion war bei keinem Spender nachzuweisen. Schlußfolgerungen: Vor dem Hintergrund der äußerst geringen Prävalenz von HTLV I/II von 0,01% erscheint die routinemäßige Testung von Blutspenden zur Zeit nicht notwendig. Objective: To define retrospectively the number of hematologic-oncologic patients who undergo platelet substitution therapy and who develop only one or few HLA antibodies over a longer period of time, as well as to determine the corresponding antigens and to evaluate the substitution principle applied: preparations from whole blood lacking the corresponding antigens. Patients and Methods: Sera of 718 hematologic-oncologic patients were tested after platelet transfusions by means of platelet immunofluorescence and lymphocytotoxicity testing. Results: Long-term single HLA antibodies were detected in approximately 5% of the patients. Transfusion responses to preparations lacking the corresponding antigens were comparable to random substitution in patients without detectable HLA antibodies as well as to compatible substitution in patients with antibodies. Conclusions: Provided that regular antibody screening is carried out and a large pool of HLA-typed donors is available, patients with few HLA antibodies can effectively be substituted with platelets obtained by whole-blood donation from donors lacking the corresponding antigens.

Background: Transmission of viral, bacterial or parasitic infections is a well-known adverse side-effect of blood transfusion. The role of medically important fungi in blood transfusion remains to be elucidated. Since enhanced numbers of colony-forming units of Candida species are being isolated from gastrointestinal and urogenital tract of healthy adults with increasing frequency, symptomless fungaemia in blood donors cannot be excluded. We therefore examined sera of healthy blood donors for serological evidence of current or previous fungal infections or presence of fungaemia. Material and Methods: By using commercially available ELISA, sera of 497 blood donors of different age were examined for presence of Candida albicans antigen and antibody. 330 sera were simultaneously screened for the presence of Aspergillus antibodies. Results: Candida albicans antigen could not be detected in any of the examined sera. 41 of 497 sera showed Candida albicans IgM antibodies, and a broad spectrum of sera had Candida albicans IgG antibodies exceeding cut-off value. Conspicuous values of Aspergillus antibodies could not be detected. Conclusions: Though most of the blood donors investigated showed serological evidence for recent or past immunological confrontation with Candida albicans, the results suggest that transmission of medically important fungi by blood transfusion will continue to be unlikely.

Ziel: Routinemäβige Erfassung und Dokumentation ent-nahmebedingter Vorkommnisse (EBV) als qualitätssichernde Maβnahme während Eigenblutentnahmen (EBS). Hilft eine Datenbank-Auswertung anhand protokollierter EBV zur Risi-koabschätzung bei Eigenblutspendern? Design: 2 stationäre Aufnahme erforderlich). Rahmen: Bereich für Autologe Hämotherapie einer Anaesthesie-Abteilung eines akademischen Lehrkrankenhauses. Teilnehmer: 898 Patienten im Zeitraum April 1993 bis März 1995, welche an einem pr-äoperativen Eigenblutprogramm teilnahmen. Ergebnisse: 2 traten bei den EBS nicht auf. Schluβfolgerungen: Behandlungsbedürftige EBV traten selten auf. Als Hauptvorkommnisse erwiesen sich Hypotension und Bradykardie, welche durch einen relativen Volumenmangel sowie vasovagale Reaktionen erklärt werden können. Prädestiniert hierfür waren vor allem weibliche Erst-spender. Durch eine qualifizierte Überwachung der Patienten während der EBS lassen sich der sogenannte Gefahrenschwer-punkt Blutentnahme entschärfen und vergleichbar niedrige Nebenwirkungsraten wie bei der
