Background: The Fresenius AS 104® cell separator has recently met some popularity in European apheresis units for the quality of the platelets obtained with the totally automated double-needle procedure. Materials and Methods: Taking advantage of the machine’s flexibility we have developed a new automated procedure for single-needle thrombocytapheresis (SNP). So far 61 SNP were carried out. From donors having a platelet precount of 2.74 ×105 an average of 3.8 ×1011 cells were collected by processing 2.5 liters of blood in a run time of 83 min. Results and Conclusions: The leukocyte and erythrocyte contaminations were 2.7×107 and 3.5×107, respectively. Only 2 products did contain less than 3.5×1011 platelets, and 7 procedures were complicated by minor signs of hypocalcemia. With minor modifications this SNP is presently offered on a routine basis to our platelet

Background: The evolution of technology has been one of the most interesting features that has accompanied thrombocytapheresis throughout the years. In this presentation we report on the preliminary results obtained with Excel®, a third-generation cell separator that is presently in its early experimental phase. Material and Methods: So far only 15 thrombocytapheresis procedures, 2 of which of the single-needle type, were carried out on 15 healthy donors processing 4 liters of blood (2.8 liters in single needle). Results and Conclusions: The results were satisfactory since the average platelet yield was 4.7×1011. The leukocyte contamination was 0.8×106 and the RBC contamination 0.4 ×107. The platelet efficiency per minute was 6.5 ×109, and the run time averaged 72 min. As to the quality of the product the very first results of platelet aggregation, relative to pre-apheresis, were 78% in response to ADP 6 µM; 106 and 102% in response to collagen 4.5 µg/ml and ristocetin 1.5 mg/ml, respectively. Hintergrund: Der zunehmende Bedarf an Thrombozytenkonzentraten erfordert aus logistischen Gründen, die Thrombozytenspende frühzeitig durchzuführen und die Thrombozyten über mehrere Tage zu lagern. Ein besonderes Problem stellt allerdings die Qualitätskontrolle derartig gelagerter Thrombozyten dar. Die bisher zur Verfügung stehenden Methoden sind zeit- und personalaufwendig und erlauben zum Teil nur Aussagen über einzelne biochemische und physiologische Parameter. Die vorliegende Arbeit beschreibt eine neuartige Methode zur Qualitätskontrolle von Thrombozytenkonzentraten. Material und Methoden: Einzelspender-Thrombozytenkonzentrate wurden über 7 Tage gelagert; täglich wurden Aliquots entnommen und die In-vitro-Blutungszeit (Thrombostat 4000®) sowie die Thrombozytenaggregation (Aggregometer) bestimmt. Ergebnisse: Der In-vitro-Blutungstest ist schnell und einfach durchführbar, die Ergebnisse entsprechen denen der konventionellen Aggregometrie. Schlußfolgerung: Der In-vitro-Blutungstest erscheint als geeignete Methode, die Funktionsfähigkeit gelagerter Thrombozyten auch im Routinelabor

A 53-year-old man with abnormally elevated cold agglutinins was treated successfully by plasma exchange so that he was able to sustain coronary artery surgery and extracorporeal circulation. Two plasma exchanges, consisting in the exchange of one plasma volume with a 5% albumin solution each, were performed in our intensive care unit with careful monitoring of vital parameters. The plasma exchanges resulted in a reduction of the titer of the cold agglutinins and of the thermal amplitude as well. With the exception of postoperative bleeding requiring re-intervention, the patient was successfully operated and could be dismissed on the 16th postoperative day in good condition.

Background: The antigen Dia is common among Mongols (5-12%), but rare in Caucasians ( < 1%). Objective: The accidental discovery of Diego A (Dia) antibodies in a South American tourist and in an elderly German patient prompted us to investigate the frequency of Dia antigens in the German population. Design: 1,352 German random blood donors, all living in Berlin or in the areas around, were tested for the presence of the Dia antigen on their red blood cells. All donors were Caucasians. Results: Dia was found in 12 individuals (0.89%).Conclusions: We think that Dia is a low-frequency antigen in the European population. The admixture of Mongol genes following wars or migration is only of low or no importance.

Ziel: Absicht der vorliegenden Arbeit ist es, die Möglichkeiten der frühen enteralen Ernährung beim kritisch kranken Patienten darzustellen. Quellen: Übersichtsartikel zu einzelnen Teilgebieten, aber auch Ergebnisse von Originalarbeiten bilden die Grundlagen dieses Beitrages. Auswahlkriterien: Zunächst werden die pathophysiologischen Grundlagen der intestinalen Nährstoffassimilation im Rahmen des Aggressionsstoffwechsels skizziert. Die sich daraus ergebenden Konsequenzen für die enterale Substratzufuhr werden besprochen. Praktische Gesichtspunkte wie Klassifikation und Differentialindikation der Nährstofflösungen, Durchführung der enteralen Ernährung sowie der Umgang mit Komplikationen der Sondenernährung bilden weitere Schwerpunkte dieser Arbeit. Ergebnisse: Unter Beachtung der Verträglichkeit kann schon früh postoperativ oder -traumatisch mit der enteralen Ernährung begonnen werden, wobei der jejunalen Substratapplikation besondere Bedeutung beizumessen ist. Schlußfolgerungen: Sowohl die Praktikabilität als auch die Effizienz enteraler Ernährungsmaßnahmen scheinen wesentlich vom Ausmaß des intestinalen Blutflusses abzuhängen. Neue Methoden zur Abschätzung dieses regionalen Blutflusses, wie z.B. die Tonometrie, müssen insbesondere bei kritisch kranken Patienten erprobt werden, um die Indikationen zur enteralen E
