Operative and nonoperative management of patients with shoulder symptomatology, particularly when impingement symptoms constitute a component of that presentation, require a comprehensive evaluation of the rotator cuff, and in particular the supraspinatus tendon anatomy and pathology, the morphology of the supraspinatus tendon tunnel, and peritendinous soft tissues. Magnetic resonance (MR) imaging represents the only means of providing this information. This article presents a clinicopathologic approach to MR evaluation of supraspinatus tendon and peritendinous pathology and impingement syndrome.

摘要:
Rotator cuff injuries in the overhead athlete are frequently seen at all levels of competitive sports. The repetitive, high magnitude forces generated at the capsuloligamentous complex, as well as at the rotator cuff itself, can result in loss of glenohumeral stability and primary as well as secondary lesions in the rotator cuff itself. The biomechanics of overhead sporting activity (i.e., pitching, tennis) explains the tremendous load demands placed on the shoulder. These forces must be clearly understood by the sports medicine specialist before proper treatment protocols can be initiated. A fundamental understanding of the biomechanics can provide important information to maximize treatments, enhance overall performance, and accelerate a more effective and expeditious rehabilitation program. The rehabilitation of these complex injuries remains a challenge and requires a methodic, systematic approach that is goal directed and designed to fulfill specific and objective criteria before progressing to the next phase of rehabilitation. Finally, the rehabilitation must be individualized and sports specific.

摘要:
Factors leading to rotator cuff tendinopathy in the competitive swimmer include overwork, shoulder joint laxity or instability, impingement, and stroke mechanics. Balanced muscle strengthening, flexibility training, avoidance of overwork when fatigued, and technique modification when necessary are fundamental to a preventative program. As well, the importance of an informed coach cannot be overstated. Treatment ranges from conservative measures such as stretching, strengthening, icing, and physiotherapy modalities, to surgical intervention in those athletes with severe tendinopathies.

Rehabilitation of the shoulder has undergone significant evolution over the past few years. This article outlines techniques in prevention and rehabilitation principles for the injured shoulder, with specific reference to overhead athletes. Additionally, we present a comprehensive rehabilitation program for the symptomatic or asymptomatic shoulder based on these principles.

摘要:
The arthroscope has significantly changed the way disorders of the shoulder have been approached, primarily because arthroscopy has facilitated and improved the ability to diagnose, understand, and treat pathologic conditions of the shoulder. This is especially true with regard to disorders of the rotator cuff. In this review, use of the arthroscope as a diagnostic tool for evaluating disorders of the rotator cuff is discussed. In addition, the arthroscopic treatment of pathologic conditions of the rotator cuff including the impingement syndrome, rotator cuff tears, calcific tendonitis, and biceps pathology is presented.
