摘要:
The goal of knee rehabilitation is to reduce symptoms after injury or surgery and return the patient to normal activity. Knee rehabilitation programs are customized to meet the individual needs of the patient, yet focus on the patient as a whole, because return to activity promotes physical and emotional health. The formulation of the rehabilitation program takes many variables into consideration and progresses in a steady manner while employing multiple therapeutic modalities to obtain the best result. The purpose of this article is to review these modalities as they relate to rehabilitation of the knee.

摘要:
Inflammation about the knee joint may occur after acute trauma or repetitive overuse. Although this inflammatory response is an attempt to repair damaged structures, it can be a source of persistent pain and permanent tissue alteration. Various therapeutic modalities and drugs have been developed to control inflammation that may be deleterious to functional outcome after knee injuries. We review the means available to attempt to achieve this goal and provide basic clinical guidelines for treatment.

There are many advantages of motion, but injured tissues must also be given the chance to repair themselves, which may require limitations in motion. This article reviews the effects of motion, loading, and immobilization on different tissues. It looks at specific effects, their causes, and makes estimations of how much motion is appropriate.

Neuromuscular control of the active and passive restraints of the knee determine normal function. Injury interferes with this normal control and results in the development of abnormal functional patterns which the patient recognizes as instability. Rehabilitation must help regain the normal function by overcoming these abnormal patterns.

摘要:
To maximize the results following an anterior cruciate ligament reconstruction, strength straining of the involved extremity is essential. However, the postoperative rehabilitation protocol must be carefully planned and progress as motion is obtained and soft tissues mobilized. Initially the strengthening program should use isometrics and closed chain exercises. As the protocol progresses, submaximal and endurance exercises are added with proprioceptive neuromuscular facilitation and functional closed chain techniques, followed by maximal resistance isometric and isokinetic exercises until full reconditioning is obtained.

Designing a rehabilitation program for the patellofemoral joint requires a thorough knowledge of its biomechanics and pathomechanics. Patellofemoral joint reaction force is directly related to quadriceps tendon force and the angle of knee flexion. Quadriceps tendon force differs greatly between functional closed-chain activities and open-chain strengthening exercises. Lateral tracking of the patella is the primary cause of dysfunction. Rehabilitation is started with reduction of inflammation and pain. Progressive training and strengthening of vastus medialis oblique function with closed-chain exercises is the hallmark of the treatment program. These exercises are complemented by stretching of tight lateral structures and correcting static and dynamic malalignment. It is important that the physician and therapist work closely together to provide a coordinated program specific for each individual patient.

Clinical and laboratory studies have altered the management of medial collateral ligament tears to include many nonoperative protocols. Early mobilization and functional rehabilitation have resulted in rapid recovery and return to athletic competition. Studies involving lateral compartment injuries remain scarce and continue to emphasize operative management and immobilization.

Rehabilitation of the anterior cruciate ligament-deficient knee seeks to return the individual to the highest possible level of function. No protocol will be universally successful for all patients, so the most appropriate rehabilitative program must be designed for each individual. We review the nonoperative and operative rehabilitation treatment options for patients with anterior cruciate ligament-deficient and reconstructed knees.
