摘要:
This article presents an overview of the status of the elderly population in nursing homes and their communication needs. The demographic profile of the older population is reviewed, and the prevalence of communication problems and nature of communicative interactions in the nursing home environment are described. Communication service availability and factors affecting service delivery are identified. Suggestions for intervention and future research and professional training needs are provided.

摘要:
Pattens of caregiver/resident communications in long-term care facilities an identified, and potential barriers to communication are highlighted through application of the Communication Predicament Model. An alternate model—the Communication Enhancement Model—is presented. This model provides a theoretical foundation for developing opportunities and strategies to enhance communication between caregiver and resident with Alzheimer's disease. A case study illustrates the positive applications of this model in the long-term care facility. Implications for research, policy, and practice are identified.

摘要:
Communication among elderly nursing tome residents is explored through a theoretical human communication framework and a review of the literature. Specific identified influences on resident talking behavior include institutional influences, residents' perceptions of rules in the musing home that restrict talking, communication motives of elders, and rules residents use to control their own talking behaviors. Six major strategies are presented to assist elderly residents in sustaining conversations in the nursing home.

摘要:
This article addresses the challenge to identify and document effective nonmedical communication interventions that will assist persons with Alzheimer's disease to function at their most optimal levels. Both direct (client-centered) and indirect (caregiver-oriented) interventions are described, and the intervention literature from disciplines other than speech-language pathology is reviewed. Emphasis is placed on functional maintenance and prevention of excessive response to disability and teamed helplessness. The article concludes with a statement of the strong need for a paradigm shift, from a focus on skills improvement to a broader quality-of-life orientation.

摘要:
Communication services for long-term care residents with Alzheimer's disease are restricted due to treatment models and assumptions, as well as reimbursement. As the severity of dementia increases, fewer communication management approaches are available. However, demands for functional interventions persist This article reviews Validation Therapy, a Rogerian intervention approach for patients in the severe stage of Alzheimer's disease. Validation Therapy addresses the patient's need to maintain contact with the environment by confirming the patient's internal emotional state, rather than coercing an external environmental orientation. Techniques and efficacy studies are reviewed; case examples of the application of Validation Therapy are provided.

Discourse analysis and intervention procedures offer promising new techniques and outcome measures for communication programming in long-term care facilities. This article reviews the nature of communication deficits observed in nursing homes and then provides an overview of discourse behaviors, tasks, levels of organization, and analysts measure. Discourse characteristics of normal elderly and of individuals with Alzheimer's disease are identified. The potential use of discourse assessments and interventions to focus communication interventions and document outcome efficacy is described, and case examples are used to illustrate discourse applications.
