摘要:
Children with cochlear implants are seen in classrooms and on caseloads in rapidly increasing numbers. Speech-language pathologists, educational audiologists, early childhood special educators, and other communication specialists are being charged with providing clinical and educational programs that will maximize auditory learning for these children. In most cases, this involves both direct service to the child and working with other educational personnel to build an auditory emphasis into the child's curriculum and daily social interaction. This article reviews research studies on the efficacy of cochlear implants for children's auditory communication skills, including speech perception, speech production and language, and it presents suggestions for building auditory skill development in children with cochlear implants based on teaching principles that have been identified as effective. Four classic levels of auditory skill development (awareness, discrimination, identification, and comprehension) are discussed, along with suggested activities that reflect each of the categories. Parameters that influence auditory task difficulty are presented. Both individual and group instructions are considered.

摘要:
The use of cochlear implants in children younger than 3 years of age presents challenges to clinicians who provide communication intervention. This article covers four of these challenges including the developmental appropriateness of materials and procedures with very young children, behavior and compliance issues, the need for less didactic instruction and more incidental learning emphasis, and recognition of the home as the primary venue for language learning. A proposed communication assessment protocol is suggested, which includes the Infant-Toddler Meaningful Auditory Integration Scale, a scale to evaluate meaningful use of sound in everyday situations. A hierarchy of intervention techniques is presented along with specific teaching activities, based upon the skills assessed.

Cochlear implants have become an important tool for the treatment of individuals with hearing impairment. A preoperative approach consisting of a multidisciplinary team that fully assesses each cochlear implant candidate will allow the selection of those patients likely to benefit. Cochlear implant technology continues to evolve including electrode design, receiver/stimulator design, and speech-processing modalities. Novel surgical techniques have enhanced the safety of cochlear implantation and have provided access to a growing number of patients. Children with congenital inner ear malformations and those with cochlear ossification are able to benefit from implantation.

摘要:
The proper programming, care, and maintenance ocochlear implants take on added importance when the users are young children. Children may not be able to communicate adequately with caregivers when the device is not functioning properly. As such, it is imperative that educators, interpreters, speech-language pathologists, and other professionals working with children who have cochlear implants in educational settings become familiar and comfortable with the devices and learn to recognize problems. This knowledge can lead to fewer and shorter periods of time when a child is without hearing during critical educational periods. Speech processing strategies, daily maintenance, and more detailed troubleshooting techniques are reviewed.

Children with severe to profound hearing loss who receive cochlear implants have an opportunity to develop the auditory skills that will assist them in lifelong learning. Although the auditory access provided by a cochlear implant can make this process easier, there remain a number of challenges that face a child with an implant, especially in the educational setting. Children with implants will still encounter acoustic challenges, academic challenges, attention challenges, associative challenges, and adjustment challenges. The knowledgeable speech and hearing professional who is aware of the potential of the implant while at the same time is cognizant of possible barriers to a child's success in school, will assist the child is using the device to receive maximal benefit.
