摘要:
A meaningful quality assurance program comes onh with appropriate knowledge, communication, and accountability for all quality assurance functions among nurses at all levels. Describing the implementation of their quality assurance program, these authors tellof their eventual realization that, in fact, they had merely implemented a quality assurance tool that brought the nursing staff little benefit and a lot of grief. Highlighting the identifiedproblems, the factors contributing to the program 5 failure, and the actions they took to develop and meet appropriate objectives, they detail their later, successful implementation of the concept and the process of quality assurance. Their experience provides readers with foodfor thought andstimulus for critical evaluations of their own quality assurance programs.

摘要:
To foster achievement of professional and organizational goals, leaders in nursing service and education must pay attention to reward mechanisms for nursing practice. Here the author describes a framework for identifying and developing effective reward mechanisms and applies it to a specific situation. Using nursing preceptorship programs as an example, she illustrates how nursing administrators can develop and implement specific reward mechanisms that increase role satisfaction for preceptors and benefit both the service and the educational institution. What would adhocracy, the organizationul dream of Alvin Toffler and proven method for modern business organizations, look like in a health care facility? Would this organizational structure facilitate appropriate patient care and job satisfaction? Would it satisfy cost concerns? Fuszard proposes adhocracy as an ideal organizational structure for both patients and staff. She examines some barriers to adhocracy and provides insights and guidelines for overcoming them. Adhocracy, as presented here, is a vision toward which strong, powerful, and knowledgeable nursing administrators might work.

This article, a companion piece to Fuszard's “Adhocracy in Health Care Institutions?” also appearing in this issue, describes a health care organization that was restructured from a traditional bureaucracy to become a functioning adhocracy. Bishop discusses in depth how adhocracy is applied at the administrative and operating levels. A dressing such critical elements as authority and responsibility, communication and coordination, and project team functioning, she recognizes barriers to adhocracy, and gives readers a real-life view of how adhocracy can work in a health care organization.

This article reports a unique approach to evaluating primary nursing practice. The authors describe the development and use of an audit tool to record primary nursing structure, process, and implementation for comparison with their hospital's established standards of primary nursing. Their discussion of how they approached the evaluation process and what they learned offers useful information for improvements in planning, implementing, and evaluating primary nursing.
