Although some of the models and practices imported from business and industry have dubious or uncomfortable application to healthcare, one of the rapidly emerging paradigms appears to fit nursing and healthcare well. The concept of theknowledge-intenseorknowledge workcompany or firm, staffed primarily by a workforce ofknowledge workers, appears to describe the healthcare organization more accurately than the traditional industrial organization model drawn from the production floor or the sales organization.

The councilor model of shared governance was implemented at the Central Arkansas Veterans Healthcare System in January 1995. The goal was to reduce, eliminate, and consolidate nursing committees, enhance communication, and increase the decision-making abilities and opportunities of staff members. A study was conducted to determine the perceptions of the nursing staff relative to the culture of the organization after implementation of the model. The authors describe the process and outcomes of the resulting performance improvement processes, organizational redesign, and outcomes of this initiative.

Patients with congestive heart failure consume a large portion of healthcare dollars. After the implementation of an outpatient inotropic infusion unit, case management, a restorative care pathway, and telemanagement at a community hospital, the number of hospital admissions, the inpatient length of stay, and overall costs decreased. At the same time, customer satisfaction and functional status increased. This article describes how the improved outcomes were accomplished.

The proliferation of information about nurse-sensitive quality outcome measures in the current data-driven healthcare environment can be overwhelming. Effective utilization of such data is an even greater challenge. In this article, the authors describe how the use of a spider diagram can serve to combine data from various sources into more meaningful information for decision making. This innovative quality report card format helps nurse executives to quickly gain an understanding of the interrelationships among existing structure, process, and outcome quality indicators, as well as to identify actual or potential quality-of-care issues.

In Part 1 of this series, the authors review the evolution of patient classification system (PCS) utilization and describe a third-generation PCS model (3PCS) that focuses on optimizing the patient-caregiver process for the outcomes of cost, health, and caregiver satisfaction. Part 2 (September 1999) will review the implementation of 3PCS in a large urban hospital. Project rationale, methods for developing and implementing the system, key system components, advantages of the new system, and obstacles to implementation will be presented.
