Summary:A century of operative case reports and series of acoustic neurinomas covering the years 1890‐1990 were reviewed with respect to surgical approach, operative mortality, and preservation of cranial nerves VII and VIII. We reviewed 127 series that included 10,967 patients. The series were chronologically subcategorized into five historical eras. Cumulative operative statistics were calculated for the major operative approaches of each era. This review provided an overview of acoustic surgery progress, and allowed the detection of trends in the operative results of different surgical approaches. Analysis of the cumulative operative statistics of the major surgical approaches in the most recent era of acoustic tumor surgery provides potential decision‐making guidelines for optimizing acoustic tumor management.

Summary:The anatomical and surgical aspects of treatment of the major anterior circulation aneurysm types are discussed. The morbidity and mortality from major historical series are provided. The management of associated intraventricular and intracerebral hematomas is considered as is data bearing on the timing of surgery controversy. The results of a personally operated series of 100 aneurysms are analyzed with respect to prognostic factors, operative considerations and outcome. Early surgery and new strategies to prevent vasospasm appear to be the best hope for the patient with a recently ruptured aneurysm.

Summary:This article provides a review of lumbar disc disease and its management. A brief history of lumbar disc disease is given followed by a summary of the anatomy, clinical considerations and radiologic investigation of this problem. Treatment options including medical management, chemonucleolysis, percutaneous discectomy, and microdiscectomy are discussed in addition to standard laminectomy and approaches to the far lateral disc herniation. Special problems such as intradural disc herniation, anatomic root variants and Tarlov's cysts are also addressed.

Summary:The medulloblastoma is a small cell neoplasm of the cerebellum which largely afflicts the pediatric population and has a marked propensity for dissemination along CSF pathways. Since the adaptation of craniospinal irradiation in the 1950's, there has been a substantial improvement in patient prognosis. Now with the addition of aggressive chemotherapy regimens both pre‐ and post‐irradiation, patients with medulloblastoma have a 5‐year survival which exceeds 60%. In this review, we discuss our current surgical management of the patient with a medulloblastoma, and the post‐operative stratification of the patient into different treatment protocols. We briefly review the relationship of the medulloblastoma to primitive neuroectodermal tumors in the central nervous system. And we summarize current research efforts aimed at increasing our understanding of the medulloblastoma. From a difficult past, neurosurgeons and physicians caring for the patient with a medulloblastoma may look forward to a promising future in which the high‐risk patient may be cured.
