Transurethral prostatectomy (TURP) has been associated with a low mortality rate and excellent results in the immediate postoperative period. Recent reports, however, suggest that TURP is associated with a higher long-term mortality and re-operation rate compared with open prostatectomy. A prospective randomized study, comparing immediate and long-term outcomes of TURP, open prostatectomy, and other therapeutic modalities, is necessary.

The majority of patients with bladder outflow obstruction continue to be treated satisfactorily by transurethral resection of the prostate (TURP). Worries about mortality, morbidity and cost, combined with the inexorable progress of surgery towards minimally invasive therapies, have produced an array of potential alternatives. Assessment of available data on techniques such as balloon dilatation, prostatic stenting, laser prostatotomy and cryosurgical prostatectomy suggests that stents may prove to be a suitable alternative to TURP in unfit patients. Longer-term follow up is lacking at present.

Hyperthermia applied to the prostate for the treatment of benign prostatic hyperplasia (BPH) is now being used after its earlier encouraging application in prostatic cancer. Most of the pioneering work was carried out by our group using 915 MHz microwaves, delivered through a transrectal probe, in the 41–44°C range, using the Prostathermer. Objective and subjective improvement in obstructive and irritative symptoms has been reported in 40–60% of the cases. Evidence indicates that this is definitely not a placebo effect. Other recently introduced machines use either the transrectal or the transurethral approach and induce higher temperatures (45–48°C) in the prostate. Because the groups were not sufficiently large or comparable and there was no prolonged follow up, we cannot at the moment draw meaningful conclusions in comparing the new generation of machines with each other or with our original technology. More time is needed to establish the optimum temperature in hyperthermia for BPH, the best and safest route to achieve this objective (whether transrectal or transurethral), how long each treatment should last, which patients should receive it, and how best they should be selected and followed. Hyperthermia has been shown to have a definite place in the non-surgical treatment of BPH. At present it may be offered in particular to those unwilling to undergo or unsuitable for operation.

The enzyme 5-reductase converts intraprostatic testosterone into the potent androgen dihydrotestosterone (DHT). Hereditary defects in 5-reductase prevent the development of benign prostatic hyperplasia. Recently, an inhibitor of 5-reductase, finasteride, has been shown to decrease prostatic volume by 20–30% and improve urination in men with established BPH.

Infravesical obstruction secondary to benign prostatic hyperplasia (BPH) is related to dynamic and static factors. The dynamic component of obstruction is related to the tension of prostate smooth muscle. The objective of this report is to summarize the literature related to the smooth muscle content of human prostate. The physiology and pharmacology of human prostate smooth muscle are also reviewed.

The tension of human prostatic smooth muscle is mediated primarily by the1−adrenoceptor. Randomized placebo-controlled double-blind studies have recently demonstrated the safety and efficacy of selective1−adrenoceptor antagonists for the treatment of benign prostatic hyperplasia (BPH). This article reviews the clinical trials, evaluating selective1blockers in BPH reported between August 1, 1990 and July 31, 1991.
