摘要:
The thermometry and histological data obtained from studies of transurethral microwave thermotherapy for benign prostatic bladder outflow obstruction demonstrate that the response is thermal-dose-dependent. Over 70% of patients can be relieved of their symptoms by using conventional transurethral microwave thermotherapy. Severe bladder outflow obstruction requires high-temperature treatment with the treatment parameters of thermo-ablation to create a cavity within the prostate. Using either protocol, treatment is delivered during a single 1 h session, on an outpatient basis, without the need for general anaesthesia.

摘要:
High-intensity focused ultrasound is a relatively new technique for the treatment of human benign prostatic hyperplasia. It is now clear that it is a safe treatment. Whether it is as effective as other treatments, such as transurethral prostatectomy, is not known. Patients in whom the site of destruction of the prostatic tissue is periurethral appear to have better results. Simple and random debulking of the prostate with focused ultrasound is not as efficient a treatment as removing the periurethral tissue.

Recent community-based studies show that the signs and symptoms attributed to benign prostatic hyperplasia are extremely common among aging men and that they have a substantial adverse effect on quality of life. Nevertheless, surprisingly few men with symptoms or impaired quality of life seek treatment.

摘要:
Symptoms of benign prostatic hyperplasia are present in the majority of men aged 60 years and older. The Benign Prostatic Hyperplasia Guidelines recommend medical treatment with finasteride or α-blockers and watchful waiting as treatment options for men with bothersome symptoms. Finasteride therapy reduces the volume of the hyperplastic prostate gland, improves the symptoms associated with bladder outlet obstruction, and halts the inevitable progression of the disease. Terazosin, a selective long acting α1-blocker, reduces bladder outlet obstruction via prostatic smooth muscle relaxation and thus improves urinary flow and symptoms in men with benign prostatic hyperplasia.

摘要:
The definitive place for prostatic stents in the treatment of benign prostatic hyperplasia has yet to be determined. Temporary, nonepithelializing stents are useful in patients with a limited life expectancy in whom the alternative is a permanent urethral catheter, and perhaps in those awaiting definitive prostate surgery. Long-term results using permanent, epithelial-covering stents indicate that, although in general results are good, retreatment rates in a number of series are high. There is no doubt that prostate stents are here to stay; with further improvements in design they may challenge traditional prostate surgery.
