Screening for prostate cancer entails a risk of overdiagnosis and possible gain in lifespan must be expected to be modest. Whether screening can reduce mortality and morbidity from porstatic cancer can only be answered in randomized studies. At present, screening programmes for the general population are not advisable.

The aggressiveness of prostate cancer needs to be carefully assessed for an accurate prognosis. There is still no accepted methodology that determines the biological aggressiveness of an individual tumor. To gain insight into this, both tumor cell biological characteristics specific for aggressive cancer cells as well as factors determining the tumor-host interaction are being investigated.

Several papers have reviewed the results of surgery in the treatment of localized prostate cancer, whereas others have reviewed the results of radiotherapy using different regimens. The role of prostate-specific antigen (PSA) after either surgery or radiotherapy has been well studied in several publications and may, in future, resolve the controversy over the best treatment for localized prostate cancer.

Initial treatment choice depends partly on the patient's preference. Tumor models elucidated genetic influence, threshold tissue levels of activity and mechanism of actions. Luteotrope hormone-releasing hormone agonists and antiandrogens are integrated in clinical practice. Imidazole derivations, 5α-reductase blockers and growth-factor antagonists offer clinical potential.

Chemotherapy has at present no part in the standard management of hormone-resistant prostate cancer because of disappointing results of clinical studies. The definition of objective response is difficult in this ‘end-stage’ patient population. The search for active drugs continues, including the use of animal models andin vitrotechniques.

The most important fact in the early diagnosis of renal cell cancer is that approximately one-half of the patients are discovered incidentally when ultrasonography (US) is performed during a non-urological evaluation. The value of the three major explorations - US, computed tomography (CT) and magnetic resonance imaging (MRI) - is well known and Doppler ultrasound and fine-needle aspiration biopsy guided by US or CT are being developed now. Until the diagnosis by tumor markers becomes effective, imaging techniques are the only techniques that can help the clinician in the diagnosis of renal cell carcinoma.
