摘要:
After transurethral and transrectal microwave hyperthermia were found to be no more beneficial in the treatment of benign prostatic hyperplasia than a sham treatment, transurethral microwave thermotherapy has emerged as a viable minimally invasive treatment alternative in men with lower urinary tract symptoms secondary to benign prostatic hyperplasia. Several well conducted, randomized, controlled trials demonstrated superiority over sham treatment. Most recent efforts are directed at identifying pretreatment outcome predictors and the best candidates for this therapy.

摘要:
One of the goals in the advancement of prostatic endosurgical resection procedures is to reduce intraoperative bleeding. Traditionally, endosurgical tissue cutting and coagulation have required separate applications of current, each specialized for the particular process. A new approach to transurethral resection of the prostate involves using loop electrodes that have greater masses and dimensions than the standard tungsten loops. This allows the use of higher cutting electrosurgical wattages and results in temperatures at the electrode surface of up to 600°F (315°C). This increased temperature creates simultaneous superficial tissue coagulation during resection and results in increased hemostasis during resection. Electrosurgical principles and technology are reviewed including the initial laboratory and clinical experience with such a device (The WedgeTM, Microvasive Inc., Nattick, Massachusetts, USA).

Using standard transurethral electrosurgical technology, electrovaporization has quickly become a popular procedure for treating benign prostatic hyperplasia. It offers standard transurethral loop-like resection or transurethral resection-like efficacy without significant morbidities associated with the latter procedure. By combining two electrosurgical effects, vaporization and desiccation, into one motion, electrovaporization has the potential to surpass the standard transurethral resection as a gold standard. This review examines the clinical and technical advances developed in optimizing the surgical outcome of an electrovaporative resection. In addition to a review of ongoing clinical investigations by several institutions, an analysis of factors affecting electrovaporization and the subsequent refinement of technology resulting in new generators, surgical techniques, and electrode configurations are reported.

Several new published studies confirm the minimal morbidity, urodynamic efficacy, and long-term durability of neodymium: yttrium-aluminium-garnet laser prostatectomy for treatment of benign prostatic hyperplasia. Considerable data are now available regarding neodymium: yttriumaluminium- garnet laser-tissue interactions in the prostate and fiber delivery system characteristics that can be used to optimize this therapy. Reports on contact laser and interstitial laser approaches have contributed significantly to formulating the clinical roles of these technologies. In early studies, the holmium: yttrium-aluminium-garnet laser wavelength demonstrated potential advantages for prostatic resection.
