ABSTRACT:Of patients patch tested at a contact dermatitis clinic between 1985 and 1986, 22.5% (379/1685) had medicament sensitivity. The prevalence in men was 22.8% (193/847) and in women, 22.1% (186/838) (difference not significant). The rate appears to increase with age, 17.9% (216/1205) for patients less than 40 years old and 34.0% (163/480) for patients over 39 years old (difference, p<0.001). The differences in prevalence between Chinese (22.8%), Malay (21.3%), Indians (20.4%), and others (33.3%) were not statistically significant. Common sensitizers included neomycin (7.8%), proflavine (7.1%), clioquinol (4.3%), colophony (3.3%), and wool alcohol (2.7%). Proflavine sensitivity was significantly more common in men (9.9%) than women (4.2%) (p<0.001). Most dermatitis occurs on the limbs (upper limbs, 44.3%; lower limbs, 43.0%). Facial dermatitis was more common in women (37.4%) than men (19.2%) (difference not significant).

ABSTRACT:Sarcoidosis is quite common in American and West Indian blacks. In America, the incidence in blacks could be up to 10 times higher than in whites. Sarcoidosis is generally believed to be rarer in African blacks and especially along the West African coast from which the American blacks trace their ancestry. The difference in incidence may be due to environmental influences, poor reporting system from inadequate local awareness of the presentation, confusion with tuberculosis, and lack of the Kveim antigen. Diagnosis of sarcoidosis is most commonly made from the pulmonary and skin manifestations. Since the skin manifestation presents readily for observation without the need for costly devices like x‐ray machines, the present study focused on and analyzed the seven cases seen in the skin clinic within a period of 2 years (an incidence of 0.85%), showing that the commonest Presentation was as scar‐sarcoid on longstanding tribal marks. Tribal scarification is common in West Africa, and one should investigate any change in a longstanding scar. ABSTRACT:Complement components C1q and C4 of classic pathway; C3d, a breakdown product of C3, and factor B of alternate pathway: and C3, a component both of classic and alternate pathways, were studied in 35 patients, comprising 18 type I (Lepra) and 17 type II (ENL) reactions. There was a significant decrease in C3 and factor B with a concomitant rise of C3d during ENL. These changes indicate their preeminent role in immunogenesis of type II (ENL) reaction. The changes in the classic pathway components, on the other hand, were insignificant, apparently suggesting its limited involvement in ENL. Furthermore, reversion of factor B and C3d after subsidence of reaction is intriguing and may indicate that they are not substantially affected even with contemporary treatment. Complement components, of both classic and alternate pathways, showed no significant alterations either during type I (Lepra) reaction or after its amelioration.
