This article reports on the challenges of measuring health status in a study that tested whether there were differences in sleep and well-being between a group of persons with HIV who reduced their caffeine intake from baseline by 90% or greater for 30 days (n= 44) versus a group of persons with HIV who continued their usual caffeine consumption (n= 44). The MOS-HIV Health Survey summary scores were used as health status measures and as covariates. While results indicated that physical and mental health status improved among experimental group subjects, several methodological considerations are offered for further HIV/AIDS researchers: (1) How should health status with various degrees of disease progression be classified? (2) How can health status from different CD4 and HIV viral load measurements be properly inferred? (3) If opting to also employ a health status questionnaire, should general or disease-specific instruments be used?

摘要:
Through individual interviews, this qualitative study examined the subjective experience of sleep quality in the lives of 6 women with HIV infection. Three major themes emerged: living differently, sleep patterns, and strategies to normalize sleep. The women in this study revealed that they were living differently than before their infection and their experience of sleep had similarly changed. Four adaptive mechanisms were used to normalize sleep: automaticized behaviors, processing alternatives, yielding to a lack of control, and vigilant self-monitoring. This article includes discussion and implications for future research.

摘要:
The methodological perspective of symbolic interactionism and interpretive research emphasizes the complexity and importance of the nurse-client relationship. In a nurse-managed stress management clinic, 18 clients were interviewed at various points during their treatment trajectory, along with 6 clinicians. Clients and providers were emphatic about the importance of a broad, holistic perspective. Spiritual aspects were discussed, but not prominently. Status differential was minimized; however, this did not make participants define the relationship as less professional. The importance of trust was a key element in the nurse-client relationship.

摘要:
Qigong is a therapeutic Chinese practice that has been used for thousands of years to optimize and restore energy (Qi) to the body, mind, and spirit. Elements of Taoist and Buddhist philosophies form the foundation of Qigong, which promotes health and vitality through gentle exercises for the breath, body, mind, and the voice. One component of Qigong is T'ai Chi, an exercise modality that uses gentle movements. Research has demonstrated that Qigong and T'ai Chi have beneficial effects on the body and that these are becoming popular holistic modalities in the United States. Nurses should understand the philosophy, concepts, and elements of Qigong and T'ai Chi so that they can guide patients to the benefits of this ancient art and science.
