The article addresses the realities of health promotion practice in a managed care environment, synthesizing the most important findings from the midrange theories currently guiding wellness-oriented research. Factors that have been shown to predict engagement in a number of health behaviors are identified and arc formulated into a guide for clinical assessment, intervention, and outcome evaluation for clients across the life span.

摘要:
Childhood chronic illness presents challenges to families in the area of health promotion. Parents are diligent in handling the concerns associated with chronicity and compromised health status but may find themselves excessively directing their energies toward the illness-oriented needs of the child rather than health. The article identifies Healthy Children 2000 objectives and discusses their application to children with chronic illness. Health risks that confront families place extra burdens on them and require adjustments in life style, coping, support systems, and finances. Specific risks focus on life style changes, nutrition and exercise concerns, developmental needs, and environmental hazards.

A holistic approach to gynecologic health addresses more than the physical problems associated with the reproductive organs and breasts. The article presents information about selected gynecologic health issues for young and middle-age women (17 to 60 years) in a context that considers other aspects of the client, such as the woman's age, life style, health behaviors, culture and ethnicity, and education. Strategies to promote gynecologic health are discussed in relation to these aspects.

Planners of sexual health promotion interventions for adolescents and young adults confront many challenges. Multiple factors are associated with high-risk sexual behavior, and its amelioration is complex. Sexual health promotion involves more than just targeted, short-term disease prevention efforts. Primary prevention programs must be well designed, long term, and research based. The article presents suggestions for primary prevention activities in light of current social cognitive theories and research-delineated risk factors.

摘要:
The health promotion needs of persons with schizophrenia have not been emphasized as a result of historical stigma, the effort required to stabilize symptoms, the relapsing nature of the disease, and the helplessness felt by caregivers. Family members and individuals with schizophrenia experience shame, grief, guilt, fear, and isolation, all of which render them less able to be proactive. Health promotion strategies that change attitudes, build self-esteem, increase insight into the illness, modify behavior, provide sources of income and access to medical care, and support companionship are necessary while research efforts seek a cure for this historically misunderstood illness.

Coronary heart disease is the number 1 killer of American men and women. Although much attention has been focused on reducing risk factors for this disease, interventions have been somewhat unsuccessful. Previous models of health promotion have centered on gathering assessment data, suggesting specific strategies for health promotion, and scrutinizing client support systems. The healthy heart promotion model proposed in the article includes these factors and emphasizes the setting in which cardiovascular health promotion activities occur. A supportive environment is crucial to decreasing the magnitude of modifiable risk factors in a cost-effective manner.

Cancer is the second leading cause of death among adults in the United States. Trends indicate that the incidence of all cancers has increased since 1973. It is currently estimated that I in 3 Americans now living will develop the disease. Cancer accounts for 11% of all health care costs, and its devastating physical and emotional consequences are far reaching. Despite these grim statistics, there is hope for decreasing this huge burden. Nearly two thirds of all cancers are related to life style choices, such as tobacco use, dietary factors, and exercise; therefore, the opportunity exists to reduce cancer risk greatly. The article presents an overview of major cancers and strategies for primary prevention of those cancers that are most amenable to risk reduction.

Nurses serving this population require knowledge of age-specific health screening and health promotion services. A case study from a geriatric nurse practitioner's practice demonstrates the complexity of these activities. A diary of such services can help nurses focus on elderly women's needs for health promotion interventions.

The dramatic changes in health care delivery present challenges to nurses in practice and to the educators who must prepare professional nurses for the uncertain world of tomorrow's managed care environment. In light of the increased emphasis on health promotion occurring with current changes in health care, the article proposes numerous creative strategies that nurse educators can use to prepare students to fulfill their health promotion responsibilities effectively. The concepts of health promotion and creativity are examined, and teaching approaches needed to facilitate community-based, health promotion-focused activities are presented. Specific strategies, particularly clinical teaching/learning opportunities, are suggested.
