Patients are often given written materials that are above their reading skill level. When this occurs, it is unlikely that the patient will understand the information. Without an understanding of the written word, it is unrealistic to expect the patient to learn or comply. Health professionals can assist in the educational process if they have the tools to assess the readability of varied educational materials. This article describes a reading assessment tool and assists the health professional in using this tool to evaluate and revise educational materials.

An implantable defibrillator is a surgically implanted device capable of continuously monitoring the patient's heart rhythm and automatically delivering a cardioverting or defibrillating shock to the heart. This article describes one approach to teaching the implanted defibrillator patient. Preoperative, postoperative, and discharge teaching goals and content are discussed. A case study illustrates the need for contact with the emergency medical system.

Sudden cardiac death is one of the major public health problems in the United States today. Although the implementation of the automatic implantable cardioverter-defibrillator has been shown to improve the survival in patients resuscitated after sudden cardiac death, it has also resulted in a new clinical population with unique psychosocial needs. A support group may assist cardiovascular nurses in meeting these psychosocial needs. This article describes the experience in developing a support group for automatic implantable cardioverter-defibrillator recipients and their families in one medical center.

摘要:
This article provides a summary of the history of antitachydysrhythmic devices and their current application in patients with ventricular dysrhythmias. Future trends are also discussed, followed by a case study illustrating the nursing care required for a patient with a combination antitachycardia pacemaker and automatic implantable cardioverter-defibrillator for slow ventricular tachycardia. The basic principles provided can be incorporated into the teaching plans of patients with antidysrhythmic devices for malignant ventricular dysrhythmias.

This study was conducted to document whether differences in psychosocial adjustment exist between cardiac arrest survivors and patients with recurrent ventricular dysrhythmias and to identify specific patient characteristics associated with psychosocial adjustment in those individuals who have experienced ventricular dysrhythmias with or without cardiac arrest. Nineteen cardiac arrest survivors and 21 ventricular dysrhythmia patients participated in the study.There was no significant difference in psychosocial adjustment between the two groups. The characteristics that were found to be predictive of psychosocial adjustment in patients with ventricular dysrhythmias were marital status, number of dysrhythmic events, and history of heart failure.

In-hospital cardiac arrest secondary to ventricular dysrhythmias following acute myocardial infarction remains a serious concern for hospital staff. One cause associated with this life-threatening problem is hypokalemia, which leads to electrical instability in myocardial cells resulting in primary ventricular fibrillation. In this article, a case study is presented that illustrates the hospital course of a patient with severe hypokalemia who experienced ventricular fibrillation immediately following admission to the emergency department.

This study compares the learning needs of patients diagnosed with coronary artery disease who experience angina with ratings by the patients themselves and the nurses who care for them. Also compared are the rankings done on categories of select learning needs with rankings done on categories of actual or intended adherence to prescribed medical regimens, using data obtained from the patients. Samples consisted of 15 telemetry unit nurses, 15 inpatients with angina, and 15 postdischarge patients with angina. All participants responded to the Cardiac Patient Learning Need Inventory and a demographic data sheet. Patients also responded to the Educator Preference Tool and either the Health Intention or Health Behavior Scale. Data analysis indicated that similarities and differences existed between patients and nurses in what they believed is important for patients to learn. Patients expressed a greater preference for physicians, rather than nurses, to teach them cardiac information. The findings indicate that what patients believe is most important to learn, as indicated by select learning needs categories, is related to what they most intend to do or actually adhere to regarding their medical regimen.
