摘要:
Considerable progress has been made over the past several decades in development of ventricular assist device (VAD) technology. Initially, failure to wean from cardiopulmonary bypass was the primary indication for VAD support. However, VAD role has expanded to include patients with ventricular failure due to myocardial infarction and patients awaiting cardiac transplantation. A greater number of nurses, both in the intensive care and general floors, are required to care for patients supported with VADs. Therefore, a self-study guide was developed to facilitate learning. Heart failure physiology, mechanical device descriptions, patient selection, potential complications, and nursing care of patients who require VAD support are reviewed.

This article focuses on information related to the physiologic aspects of the TCI mechanical left ventricular assist device (LVAD). The two types of LVADs designed by ThermoCardiosystems, Inc., Woburn, Massachusetts, are discussed, but most of the information provided relates to the pneumatic TCI LVAD. The structure and function of the two LVADs, including device similarities and differences, are discussed first. Then physiologic, device, and treatment variables that influence the nursing care of patients requiring the pneumatic TCI LVAD are presented. Pneumatic console operator role and qualifications used at Sharp Healthcare and suggestions for future nursing research involving the TCI LVAD are provided.

The concept of intraaortic balloon counterpulsation (IABC) was developed in the late 1960s, primarily for use in cardiogenic shock. Since that time, many published reports have documented the efficacy of this mechanical assist device in an expanding number of clinical situations. Research has shown that intraaortic balloon counterpulsation increases coronary flow velocity and decreases afterload, thus favorably affecting myocardial oxygen supply and demand. This article offers a review of the concepts and mechanics of IABC, as well as current indications and contraindications for use. The nurse must be able to recognize and address potential complications to ensure positive patient outcomes.

Right ventricular failure (RVF) is manifested by a low cardiac output state that is potentially lethal. Pulmonary artery balloon counterpulsation is a means of treating RVF that is refractory to conventional pharmacologic interventions. This treatment is rarely used in isolation but in conjunction with mechanical circulatory support of the left ventricle.

Intensive care unit nurses are often faced with challenging situations, This case study focuses on an older school-age child in an adult intensive care unit (ICU) for the placement of a biventricular assist device (BIVAD) and eventual heart transplantation. Discussion of the assist device and transplantation is included as well as a discussion of the nursing care issues. The care issues discussed include alteration in family processes, alteration in body image, and grieving related to the possibility of death.

摘要:
The Safe Medical Devices Act of 1990 and subsequent federal Food and Drug Administration (FDA) regulations provide a new dimension in the regulation of health care institutions. For the first time, the FDA has the power to require many health care facilities to investigate, document, and report serious events related to all medical devices, from ventricular assist devices, to catheters, to sutures. The FDA also has the power to require facilities to track certain devices from receipt, through patient use, to disposal. Staff must be trained, procedures implemented, and compliance achieved. Lack of compliance may carry civil and criminal penalties in the future, affect liability and risk management, and influence accreditation.
