ABSTRACTA telephone survey of all non‐governmental obstetricians, family physicians, general practitioners, and osteopathic physicians in rural Arizona was undertaken to determine the effect of medical liability issues on the availability of rural obstetrical services. One hundred ninety‐one (88.8%) responded, and after exclusion of those who had never provided obstetrical care, 126 physicians remained for evaluation. These included 32 obstetricians, 55 family physicians, 25 general practitioners, and 14 osteopaths.During the past three years, 26 (20.6%) had discontinued providing obstetrical services, citing liability issues as the reason. An additional 12 physicians (9–5%) planned to discontinue obstetrics upon expiration of their 1986 malpractice insurance policy. By the end of 1986, the number of obstetrical providers in rural Arizona will have decreased by 30.1 percent.Women in many rural areas already have pregnancy outcomes that are inferior to their urban counterparts. ABSTRACTDeciding on a system for keeping medical records in a new rural health practice, or in an established practice seeking to update an obsolete system, may be confusing because of a multitude of vendors who claim to meet every need. Special circumstances set rural health practices apart from other ambulatory care settings with regard to medical record needs. Practical considerations regarding filing systems, record content and format are offered in view of these special circumstances. Issues involving ownership of the record and access to confidential information are also explored.

ABSTRACTTwo hundred and fifty five heads of households were surveyed in Iyekuselu district, Bendel state, Nigeria. Fifteen villages were randomly selected from the list of 107 villages that constitute the district. A total of seventeen heads of households were interviewed in each of the fifteen villages. This survey was conducted to assess the attitudes of subjects toward modern medicine and traditional medicine. The heads of households generally expressed positive attitudes toward the health services provided at the modern medicine facilities. Some of them also reported accepting traditional healing administered by native doctors as legitimate alternative health care. For the illnesses considered in this study, the majority of the respondents were willing to consider modern medicine.

ABSTRACTNurses employed in ten rural hospitals in Canada completed questionnaires designed to examine their perceptions of the strength of the professional relationship which existed among nurses and between nurses and physicians. Four questions addressed the availability of nursing education programs and the attendance of nurses and physicians at such programs. Seven questions examined the nurses’perception of physician interest in nursing education and the encouragement received from their nursing colleagues to participate in learning activities and medical management decision making. Chi square analysis was undertaken to determine whether the colleagial relationships differed by hospital or medical staff characteristics. Forty percent of the nurses felt that the majority, of physicians were supportive of continuing nursing education. Sixty‐three percent indicated their nursing colleagues encouraged them to attend educational programs. The results indicate that physician participation in nursing continuing education was greatest in communities with the fewest health resources (an acute hospital with fewer than 50 beds). In contrast, encouragement to participate in continuing nursing education was highest in settings where the hospital provided both acute and extended care services, had more patient admissions and a greater availability of educational programm

ABSTRACTThis retrospective comparative study assessed the levels and types of stressful life events among rural versus urban women in Wyoming (n = 157). No significant difference in total life events scores, negative life events scores, positive life events scores, or total number of life events reported was found between the rural and urban samples. Rural women tended to report environmentally related stressors more frequently than urban women, while urban women reported more financially related stressors. The findings suggest that the levels of stress among rural and urban women are comparable, but the types of stressors experienced may vary.

ABSTRACTIn this paper, the rural/urban geographic distribution of licensed health professionals living in the state of Georgia is examined. Using 1983 data from the State Examining Boards, 13 health professions were studied to determine rural and urban differences. Three indicators of ruralness were used to classify county of residence: (1) metropolitan/nonmetropolitan, (2) metropolitan proximity, and (3) size of county population. Results from data analyses indicate a severe geographic maldistribution of health professionals. With all three indicators, 11 of the 13 professions had urban rates of professionals‐to‐population substantially higher than the corresponding rural rates. A linear configuration seems to illustrate the relationship between the availability of health professionals and the ruralness of the county—as the county becomes more rural, the number of health professionals per population decreases. The percentage increase in physicians since 1968 and nurses since 1979 in nonmetropolitan counties was slightly higher than in metropolitan counties, indicating that this gap is closing somewhat. The most severe rural/urban differences in the number of professionals which per population were found in the more specialized health professions per tended to be the smallest in terms of numbers of members (e.g., occupational therapists, psychologists, speech pathologists/audiologists, podiatrists, opticians and physical therapists). These differences ranged up to a 20 plus‐fold difference.
