Proteoglycans and glycosaminoglycans in normal gingival and phenytoin‐induced gingival overgrowth were studied by gel electrophoresis and HPLC methods after extration with guanidinium hydrochloride and subsequent cesium chloride gradient centrifugation. The results showed that normal gingival. The relative collagen content was decreased in the phenytoin lesion. Basal cells were immunocytochemically localized with a monoclonal anti‐cytokeratin antibody in the human masticatory cpithelium. The antigen was also found in the junctional epithelium and in the epithelial rests of Malassezin vivo, as well as in the cells growing along a Milliporeafilter under a mucosal explantin vitro. The equivalent expression of the basal cell specific antigen in the cells adjacent to the toothin vivoand along the filter in culture suggests that inert substrates as such or through the extracelluiar matrix synthesized in between, have an influence on the course of epithelial differentiation. This was further supported by the finding that the cells in contact with the filter attained basal cell appearance and Incorported3H‐thymidine. A 3‐month clinical trial was conducted to compare the effects of two concentrations of a chlorhexidine mouthrinse on gingivitis and plaque accumulation. Six hundred (600) adults were divided into three treatment groups matched for age, sex, and initial gingivitis severity. Following a thorough examination and prophylaxis at baseline, subjects were given a mouthrinse containing either 0.12% or 0.20% chlorhexidine gluconate or a placebo mouthrinse. Subjects were evaluated after 6 weeks and 3 months of mouthrinse use. After 3 months, both chlorhexidine groups showed significantly less gingivitis and plaque than the placebo group. The group using the 0.12% chlorhexidine gluconate mouthrinse demonstrated 27–27 % less gingivitis occurrence and 28–28% less gingivitis severity than the placebo group. The 0.12% chlorhexidine gluconate group also had 48–48% less gingival bleeding and 36% less plaque than the placebo group. There were no significant advantages for the 0.20% mouthrinse over the 0.12% mouthrinse. To determine the effect of altering the nature of the surface of the roots of teeth upon selective attachment of specific cell types, we have studied the binding of fibronectin (FN) and laminin (LN) to both undemineralized and demineralized root surfaces using a modified direct ELISA. Discs of dentine with the cementum removed were prepared from porcine premolar roots and incubated with various concentrations of FN or LN. The amount of attachment protein that binds to the dentine surface was determined using specific antibodies. It was found that FN binding was two‐fold greater on demineralized compared to undemineralized root surfaces. Healing the demineralized discs increased FN binding, whereas bacterial collagenase treatment essentially abolished binding, suggesting that FN binding occurred to exposed collagen fibres. In contrast, FN binding on undemineralized discs was unaffected by heat or collagenase treatment. Pronase treatment eliminated FN binding to demineralized and to undemineralized discs, whereas 4M guanidine hydrochloride extraction increased binding lo undemineralized discs, indicating that a non‐collagenous protein on Ihe undemineralized surface may bind to FN, Studies with LN showed a two‐fold greater binding of LN to undemineralized compared to the demineralized roots. The effects of a chlorhexidine gluconate mouthrinse on plaque and gingivitis were studied among 430 adults in a six‐month clinical trial. Subjects were divided in two comparable treatment groups matched for age, sex, and initial gingivitis severity. Following a thorough oral prophylaxis, they were instructed to rinse twice a day for 30 seconds with 15 ml of a 0.12% chlorhexidine gluconate mouthrinse or a placebo mouthrinse. After three and six months of mouthrinse use, the chlorhexidine group had significantly less gingivitis, gingival bleeding, and plaque accumulation compared to the placebo group. As expected, accumulation of dental calculus and extrinsic dental stain increased in the chlorbexidine group. No significant differences in adverse oral soft tissue effects were observed between the two groups.Since use of oral antimicrobial agents has been reported to produce tooth and tongue stain, gingivitis examinations were done with and without tooth covers to eliminate the potential for examiner bias. The present study was undertaken to compare the calibration curve of the periotron* (HAR‐6000) with those of two earlier models (HAR‐600) instrument, which was found to be appreciably more sensitive in detecting small volumes of fluid than the two older models, is curvilinear in shae while the latter two models display linear calibration characteristics Calibration curves were also obtained for one of the HAR‐600 models with the periopaper strip inserted up to and beyond the manufactures recommendations. The antimicrobial effect of a 0.12% chlorhexidine gluconate mouthrinse on plaque bacteria was compared to that of a placebo mouthrinse without chlorhexidine gluconate. Forty subjects in each treatment group rinsed twice daily with 15 ml mouthwash for six months. They were instructed to continue their usual oral hygiene practices. After six months, the subjects stopped using the mouthrinse. Supragingival plaque was collected from each subject and assayed for eight representative microbial populations at baseline, after three and six months of treatment, and after a three‐month post‐treatment period.During the six months of mouthrinse use, significant reductions in the numbers of total aerobes. total anaerobes, streptococci, and actinomyces were observed in plaque samples taken from subjects using the chlorhexidine mouthrinse. The effect of chlorhexidine on actinomyces accumulation was especially pronounced with reductions ranging from 85–97%. This was considered to be particularly significant since actinomyces is one of the bacteria known to be periodontopathic in both experimental animals and in humans. Although chlorhexidine treatment reduced the number of microbes on teeth, it did not produce a detectable shift in microbial populations.At the end of the three‐month post‐treatment period, the microbial profile was similar to the pre‐treatment profile for both groups.
