The precise pathogenesis of one of the most common cutaneous disorders, diaper dermatitis, has not yet been completely established, perhaps in part because relatively little histopathological knowledge has been obtained. This paper summarizes the available information in this field, mostly obtained by early investigators.

Biopsies of clinically unaltered skin from a 5‐year‐old girl with metachromatic leukodystrophy were studied by light, fluorescence and analytical electron microscopy. Investigation of hematoxy‐lin‐eosin‐stained sections revealed no pathologic changes, but a brown metachromatic material was found within cutaneous nerves after acetic acid cresyl violet staining of frozen sections. In semithin Epon sections small dermal nerve fascicles contained endoneural deposits, which proved to correspond with the typical prismatic, dense or Tuffstein bodies described in other organs of patients suffering from MLD. The inclusions were located mainly within Schwann cells and exhibited a bright orange fluorescence after trypaflavine‐phosphotungstic acid‐treatment as well as a high electron density in serial ultrathin sections. Energy dispersive X‐ray microanalysis revealed a high sulfur content in the membrane bound granules. These findings demonstrate the presence of the specific storage material also within cutaneous nerves in MLD and thus suggest skin biopsies as an additional and simple diagnostic aid

Tinea versicolor (TV) infections were studied in the scanning electron microscope utilizing several different sampling and preparatory techniques. Scalpel scrapings provided information only on the morphology of individual spores and hyphae, while biopsies and cyanoacrylate adhesive slides demonstrated the topography of the disease as well as providing information on how the infection spreads and recurs. Critical point drying was necessary to maintain the fine ultrastructure previously described. The deep follicular location of the spores seen in this study probably explains the tendency for TV to recur. The follicular packing was specific, since the eccrine pores seem to be exempt from the infection.

Histologic, immunologic and electron microscopic studies were performed in a patient with transient acantholytic dermatosis which involved oral mucosa. Histologic and electron microscopic findings were almost identical in both cutaneous and mucous membrane lesions, and these were similar to pemphigus vulgaris; suprabasilar separation with acantolytic cells. Desmosome‐desmosome complexes were separated without disruption of cell membranes and cytoplasmic structures were well‐preserved without dyskeratosis. Lamina lucidia, however, was often separated in mucous membrane lesions, in contrast to the normal lamina lucida in cutaneous lesions of such cases or in pemphigus vulgaris. Direct and indirect immunofluorescence studies for pemphigus were repeatedly negative. A bone marrow needle biopsy was performed in 19 patients with mycosis fungoides (MF). Two patients, one previously classified as stage 4 and the other with diffuse erythroderma had pathological bone marrow findings. Seventeen patients showed no evidence of bone marrow involvement, 10 of these were in the plaque stage. Bone marrow biopsy can be of value in assessing the stage of the disease.

摘要:

This report describes eccrine sweat gland involvement in a patient with GM1gangliosidosis. A striking vacuolization was noticed in cells of the acrosyringium and of the secretory coils. The diagnostic value of a skin biopsy in this condition is emphasized.
