SUMMARYExperimental hydrocephalus was induced by the infusion of a non‐inflammatory silicone oil into the cisterna magna of adult rabbits and infant dogs. In an histological and electron‐microscopical study, the brain tissue damage in these animals was compared with that in cerebral biopsies from hydrocephalic human infants.The main features of ependymal disruption, with nerve fibre destruction and clear‐fluid (probably csf) extracellular oedema of the periventricular white matter, were present to varying degrees in all the severe hydrocephalic brains. Very much less disruption occurs in the unmyelinated infant brain than in the partially or fully myelinated animal brains. It appears that the extent of the tissue damage bears some relationship to the degree of white matter myelination.RÉSUMÉUne hydrocéphalie expérimentale a été provoqutée par l'injection d'huile de silicone non inflammatoire dans la grande citerne de lapins adultes et de chiots. Une ètude histo‐logique et électromicroscopique a permis de comparer les lésions cérébrales chez ces animaux avec les biopsies cérébrales provenant de jeunes hydrocéphales humains.Les principales caractéristiques de l'épanchement épendymaire, avec destruction des fibres nerveuses et oedème de liquide clair (probablement LCR) de la substance blanche périventriculaire, ont été observées á des degrés variés dans tous les cas d'hydrocéphalies sévères. Un épanchement beaucoup moindre se produit dans le cas du cerveau non myélinisé du jeune enfant que dans les cerveaux animaux, partiellement ou totalement myilinisés. II apparait donc une relation entre l'étendue des lésions tissulaires cérébrales et le degré de myélinisation de la substance blanche.ZUSAMMENFASSUNGEin experimenteller Hydrocephalus wurde durch die Infusion von nicht entzündendem Siliconöl in die Cysterna magna von erwachsenen Kaninchen und jungen Hunden hervorge‐rufen. In einer histologischen und elektronenmikroskopischen Untersuchung wurde die Schädigung des Gehirngewebes verglichen mit der in cerebralen Biopsien von hydro‐cephalischen Kindern beobachteten Schädigung.Die hauptsächlichen Merkmale der ependymalen Zerreiβung mit Zerstörung von Nervenfasern und einem klarüssigen (wahrscheinlich CSF) extrazellularen Oedem der periventrikulären weiβen Substanz fanden sich in unterschiedlichem Ausmaβ in allen schwer hydrocephalischen Gehirnen. In den nicht myelinisierten kindlichen Gehirnen kommt sehr viel weniger Zerreiβung vor als in den teilweise oder ganz myelinisierten Gehirnen der Tiere. Es scheint so, daβ das Ausmaβ der Gewebeschädigung in einem gewissen Verhältnis zur Myelinisierung der weiβen Substanz steht.RESUMENSe produjo una hidrocefalia experimental por la infusión de un aceite silicónico ininflam‐able en la cisterna magna de conejos adultos y de perrillos lactantes. En un estudio histológico y electromicroscópico, se comparó el daño a los tejidos cerebrales en estos animales con el daño revelado por biopsias cerebrales en lactantes humanos con hidrocéfalia.Las características más importantes de disrupción ependimaria, con destrucción de las fibras nerviosas y edema extracelular de líquido claro (probablemente líquido cefalorra‐quídeo) de la substancia blanca periventricular se hallaban a un grado variable en todos los cerebros hidrocéfalos más graves. SUMMARYA survey of all children born with open myelomeningoceles in the city of Liverpool between the years 1960 and 1965 is presented. Over 80 per cent of the children who survived the first 24 hours came to surgery and 70 per cent of these children are now surviving to school age and beyond. An analysis is made of their physical and mental handicaps.RÉSUMÉLes auteurs présentent une étude de tous les enfants nés avec un myéloméningocèle ouvert, à Liverpool, durant les années 60 à 65. SUMMARYThe results are reported of a controlled trial in the treatment of hydrocephalus with ventriculo‐cerdiac shunts in the first week of life of infants born with spina bifida cystica or cranium bifidum.The most important conclusion which the author draws from this trial is that infants with less severe degrees of hydrocephalusmust notbe operated on early, as it was found that many of them will either not need the operation at all, or will need it only at a much later date, with greatly reduced risk of complications and need for revisions. Very early operation should only be considered if there is already a severe degree of ventricular dilatation in the first week of life.RÉSUMÉOn rapporte les résultats d'un contrôle prolongé du traitement de l'hydrocéphalie par shunt ventriculo‐cardiaque réalisé au cours de la premièe semaine chez des enfants nés avec spina bifida ou cranium bifidum. La conclusion la plus importante de ce contrôle est que les enfants présentant une hydrocéphalie marquéene doivent pasětre opérés aussi tôt, du fait que beaucoup d'entre eux se passeront finalement d'une intervention ou la verront retardée à une époque plus éloignée, avec eliminution très importante du risque de complications et du besoin de réintervention. Les opérations très préoces ne doivent être envisagées que s'il existe déjà au cours de la première semaine une dilatation ventriculaire importante.ZUSAMMENFASSUNGEs wird über die Ergebnisse einer kontrollierten Behandlungsserie des Hydrocephalus mit ventriculo‐cardialen Shunts in der ersten Lebenswoche von Kindern berichtet, die mit Spina bifida cystica oder Cranium bifidum geboren wurden. Das wichtigste Ergebnis dieser Serie bestand darin, daß Kinder mit weniger schwerem Hydrocephalusnichtfrüh operiert werden müssen, da man fand, daß viele von ihnen entweder gar nicht oder zu einem viel späterenzeitpunkt mit erheblich reduziertem Komplikationsrisiko und geringerer Revisions‐rate operiert werden müssen. SUMMARYA follow‐up study was made of 67 former pupils of a special school for children suffering from cerebral palsy. Twenty‐four of these were gainfully employed and 16, who were still in training, seemed likely to become so. Sixty per cent of these former pupils could therefore be considered employable. Of the patients with hemiplegia, 83 per cent were working, all in open employment; by contrast, only 44 per cent of the diplegic patients and 36 per cent of the dyskinetic patients were working and only 28 per cent and 14 per cent respectively were in open employment.The main obstacles to employment were physical disability and poor intellectual endowment. Multiplicity of handicaps was less important in this respect, as it usually occurred in patients who were prevented from working by the severity of their physical handicap.The expenditure of large sums of money on the education of severely handicapped children can be justified on humanitarian grounds alone.RÉSUMÉRevue des résultats à long terme dans la réadaptation de67enfants atteints d'infirmitéeas motrice cérébrateUne observation prolongée a été poursuivie chez 67 anciens èlèves d'une école spéciale pour enfants atteints d'infirmité motrice cérébrale.Sur ces 67 malades, 24 sont utilement employés professionnellement et 16 encore en apprentissage, semblent pouvoir le devenir dans le futur.Ainsi 60 pour cent des anciens èlèves devraient pouvoir etre considérés comme employables.Parmi les malades présentant une hémiplégie, 83 pour cent sont au travail, tous en milieu ouvert. Par opposition, 44 pour cent seulement des malades diplegiques et 36 pour cent des malades diskinétiques travaillent; parmi eux respectivement 28 pour cent et 14 pour cent travaillent en milieu ouvert.Les principaux obstacles à l'émbauche ont été naturellement le handicap physique et les possibilityés intellectuelles limitées. Von den Patienten mit einer Hemiplegie arbeiteten 83 Prozent–und zwar in einem freien Arbeitsverhältnis, dagegen waren von den Patienten mit einer Diplegie nur 44 Prozent und von den dyskinetischen Kindern nur 36 Prozent arbeitsfähig und von diesen waren wiederum nur 28 prozent bzw 14 Prozent in einem freien Arbeitsverhältnis beschäftigt.Das Haupthindernis bei einer Anstellung bestand natürlich in der physischen Unfähigkeit und in der geringen geistigen Begabung. Das Ausmass der Beeinträchtigungen war in dieser Hinsicht weniger wichtig, als es gewöhlich bei Patienten der Fall ist, die so schwer geschädigt sind, dass sie nicht arbeiten können.Der Aufwand grosser Geldsummen für die Erziehung schwer geschädigter Kinder kann allein aus menschlichen Gründen gerechtfertigt werden, er würde jedoch auch grössere wirtschaftliche Berechtigung haben, wenn die Erziehung auf die Möglichkeiten einer Unterbringung, Ausbildung und Anstellung körperbehinderter Schulabgänger ausgerichtet wäre.RESUMENUn repaso de los resultados del cuidado a largo plazo en67niños afectos de una parálisis cerebralSe estudiaron 67 enfermos que habfan sido alumnos de una escuela especial para niños afectos de una parálisis cerebral.De estos 67 enfermos, 24 trabajaban por dinero, y parecía probable que otros 16, todavía bajo instrucción, trabajasen también. Un 60 por ciento de estos ex‐alumnos, pues, se podían considerar capaces de trabajar. Entre los hemipléjicos, un 83 por ciento trabajaban, todos en empleos no protegidos; en cambio solamente un 44 por ciento de los dipléjicos y un 36 por ciento de los disquinéticos trabajaban y de éstos sóalo un 28 por ciento y un 14 por ciento respectivamente tenían empleos no protegidos.Las dificultades principales que se oponían al trabajo eran naturalmente incapacidades fisicas y mentalidad pobre.
