SUMMARY103 consecutive childhood cases of genetic peripheral neuropathies of heredodegenerative background were collected from Gothenburg from 1973 to 1980. From this series, 63 hereditary motor and sensory neuropathies (HMSN) were distinguished: 31 cases of demyelinating and remyelinating HMSN (HMSN I), 21 (18 families) with an autosomal dominant and 10 with sporadic mode of inheritance and unaffected parents; and 32 cases of neuronal‐axonal types (HMSN II), 27 of whom (25 families) had at least one affected, if asymptomatic, parent. In one family, both parents were neurologically and neurophysiologically completely normal. Three cases of uncharacteristic HSN were diagnosed. Among 37 cases with a combined degenerative encephalopathy/myelopathy and a peripheral neuropathy, nine had hereditary spastic paraplegia, six had heredoataxias (three of the Friedreich type), nine had lysosomal storage diseases (five of the Krabbe type), seven had other known inborn metabolic errors and six had biochemically undefined disorders. Progressive neuropathies are important manifestations of a large variety of genetically determined heredodegenerative neurological disorders of infancy and childhood. For classification of HMSN, clinical and neurophysiological examinations are necessary for the index case and for both parents as well.RÉSUMÉNosologie des neuropathies périphériques d'origine génétique chez les enfants suédois103 cas consécutifs de neuropathie périphérique de l'enfance d'origine génétique dans un contexte hérédodégénératif ont été recueillis à Gothenburg entre 1973 et 1980. A partir de cette série, 63 neuropathies héréditaires motrices et sensorielles (HMSN) ont été remarquées: 31 cas de HMSN démyélinisante et remyélinisante (HMSN 1), dont 21 (18 familles) avec un caractére dominant autosomal et 10 avec un mode sporadique de transmission et parents non atteints; et 32 cas de type neuronal‐axonal (HMSN 2), parmi lesquels 27 (25 familles) avaient au moins un parent atteint même si l'atteinte était asymptomatique. Dans une famille les deux parents étaient tout à fait normaux neurologiquement et neurophysiologiquement. Trois cas de HSN non caractéristiques ont été diagnostiqués. Parmi 37 cas associant une encéphalopathie dégénérative, une myélopathie et une neuropathie périphérique, neuf présentaient une paraplégie spastique héréditaire, six une hérédoataxie (dont trois du type Friedreich), neuf une thésaurismose lysosomale (dont cinq du type Krabbe), sept présentaient d'autres défauts métaboliques innés connus et six des désordres biochimiques non définis. Les neuropathies progressives sont donc des manifestations d'une grande variété de troubles neurologiques hérédo‐dégenératifs de la petite enfance et de l'enfance à détermination génétique. La classification des HMSN exige un examen clinique et neurophysiologique à la fois du sujet et de ses parents.ZUSAMMENFASSUNGDie Nosologie der genetischen peripheren Neuropathien bei Kindern in SchwedenIn Göteburg wurden von 1973‐80 103 kindliche Fälle mit genetischer perihperer Neuropathie von heredodegenerativem Ursprung zusammengetragen. In dieser Serie waren 63 hereditäre motorische und sensorische Neuropathien (HMSN): 31 Fälle mit demyelinierender und remyelinierender HMSN (HMSN I), davon 21 (18 Familien) mit einem autosomal dominanten Erbgang und 10 mit einem sporadischen Erbgang und gesunden Eltern; und 32 Fälle vom neuronal‐axonalen Typ (HMSN II), davon hatten 27 (25 Familien) mindestens einen betroffenen, wenn auch asymptomatischen, Elternteil. In einer Familie waren beide Eltern neurologisch und neurophysiologisch völlig unauffällig. Es wurden drei Fälle mit uncharakteristischer HSN diagnostiziert. Von 37 Fällen mit einer kombinierten degenerativen Encephalopathie, Myelopathie und einer peripheren Neuropathie hatten neun eine heriditäre spastische Paraplegie, sechs eine Heredoataxie (drei vom Typ Friedreich), neun eine lysosomale Speicherkrankheit (fünf vom Typ Krabbe), sieben hatten andere bekannte angeborene metabolische Defekte und sechs hatten eine biochemische nicht zu definierende Erkrankung. Progressive Neuropathien sind wichtige Manifestationen einer großen Palette genetisch determinierter, heredodegenerativer neurologischer Erkrankungen im Säuglings‐und Kindesalter. Zur Einteilung der HMSN sind klinische und neurophysiologische Untersuchungen der Patienten, sowie beider Eltern erforderlich.RESUMENNosología de las neuropatias periféricas genéticas en niños suecosSe reunieron 103 casos infantiles consecutivos de neuropatías periféricas neuropáticas con un fondo heredodegenerativo en Goteborg, desde 1973 a 1980. A partir de estas series se distinguieron 63 neuropatías motoras y sensitivas (NHMS): 31 casos de NHMS desmielinizante y remielinizante (NHMS I), 21 (18 familias) con una forma autosómica dominante y 10 con herencia esporádica y padres no afectados; y 32 casos con un tipo neuroaxonal (NHMS II), de las que 27 caos (25 familias) tenían un padre o madre por lo menos afectado. En una familia ambos padres eran neurológica y neurofisiológicamente completamente normales se diagnosticaron tres casos de NHS no caracteristica. Entre 37 casos con una combinación de encefalopatia degenerativa, mielopatía y neuropatia periférica, nueve tenían una paraplegia espástica hereditaria, seis tenian una heredoataxia (tres del tipo Friedreich), nueve una enfermedad de depósito lisosomal (siete del tipo de Krabbe), siete tenian otros tipos de errores congénitos del metabolismo y seis tenian una alteración bioquímica inidentificable. SUMMARYClinical data on 24 patients with hereditary motor sensory neuropathies, with onset in the paediatric period, and of their relatives, is reported. Electrophysiological studies were done in all patients and in 15 relatives. The patients were divided into two groups (Types I and II) and their hereditary trait was determined. In 11 patients a sural nerve biopsy was performed and revealed different patterns of histological alterations. The nerve biopsy always confirmed the value of conduction velocity in distinguishing between Types I and II. A genetic discordance was observed, both in regard to the phenotype and the conduction velocity, and there was increased slowing of the conduction velocity as individuals grew older. Thus the classification of these disorders in childhood can be particularly difficult. The rôle of sural nerve biopsy is discussed.RÉSUMÉNeuropathie héréditaire motrice et sensorielle de l'enfanceL'article rapporte les données cliniques observées chez 24 malades présentant des neuropathies héréditaires sensori‐motrices ayant débuté durant l'enfance, ainsi que chez les membres de leur famille. Des études électrophysiologiques ont été faites chez tous les malades et chez 15 apparentés. Les malades ont été divisés en deux groupes (Type I et type II) et leur caractére héréditaire a été déterminé. Une biopsie du saphène externe a été réalisée chez 11 malades et a révélé des altérations histologiques diverses. La biopsie du nerf a toujours confirmé la valeur de la vitesse de conduction pour faire une distinction entre les Types I et II. Une discordance génétique portant sur le phénotype et la vitesse de conduction et un accroissement du ralentissement de la vitesse de conduction avec l âge ont été observés. Aussi la classification de ces troubles de l'enfance peut être particulièrement difficile. L'intérêt de la biopsie du saphène externe est discuté.ZUSAMMENFASSUNGHereditäre motorische und sensorische Neuropathien im KindesalterEs wird über die Daten von 24 Patienten mit heriditärer motorischsensorischer Neuropathie mit einem Krankheitsbeginn im Kindesalter und von deren Verwandten berichtet. Die Patienten wurden in zwei Gruppen eingeteilt (Typ I and II) und es wurde der Erbgang bestimmt. Bei 11 Patienten wurde eine Suralisbiopsie entnommen, die verschiedene histologische Veränderungen zeigte. Die Nervenbiopsie bestätigte in jedem Fall die Aussage der Leitungsgeschwindigkeit, durch die in Typ I und II unterschieden wurde. Es wurde eine genetische Discordanz hinsichlich des Phänotypus und der Leitungsgeschwindigkeit, sowie eine zunehmende Verlangsamung der Leitungsgeschwindigkeit bei älter werdenden Personen beobachtet. Die Einteilung dieser Erkrankungen im Kindesalter kann daher besonders schwierig sein. Die Bedeutung der Suralisbiopsien wird diskutiert.RESUMENNeuropatia motora y sensitiva hereditarias en la infanciaSe aportan los datos clínicos de 24 pacientes con neuropatía motor sensitiva iniciada en la edad pediátrica, así como los datos de sus familiares. En todos los casos se realizaron estudios electrofisiológicos así como en 15 familiares. Los pacientes fueron divididos en dos grupos T (Tipo I y Tipo II) y se determinaron sus caracteristicas hereditarias. En 11 pacientes se realizó una biopsia del nervio sural que reveló diversos esquemas de alteración histológica. La biopsia del nervio siempre confirmó el valor de la velocidad de conduccion nerviosa en la distincion entre los Tipos I y II. SUMMARYThree children, two siblings and one unrelated child, with congenital muscular dystrophy with central nervous system (CNS) involvement are discussed. The siblings appeared to suffer from a relatively mild myopathy with progressive brain disease, of which brain biopsy in one showed astrocytic proliferation in the white matter. In the patient with severe muscle disease, autopsy showed widespread patchy demyelination in the white matter and developmental abnormalities in the cerebral and cerebellar cortex. These patients differ from the Japanese (Fukuyama) cases of CMD in the severity of the changes in the cerebral white matter, and from Santavuori's cases in the absence of ocular abnormalities and hydrocephalus. Their unique nosology is discussed.RÉSUMÉAtteinte du système nerveux central dans les myopathies congénitalesL'article décrit trois enfants, dont deux apparentés, présentant une myopathie congénitale avec atteinte du systéme nerveux central (CNS). Les enfants apparentés semblaient avoir souffert d'une myopathie relativement modérée avec une atteinte cérébrale dégénérative, la biopsie cérébrale dans un cas montrant une prolifération astrocytique dans la substance blanche. Chez le dernier patient qui présentait une atteinte musculaire grave, l'autopsie a révélé une démyélinisation en plaques largement diffusée dans la substance blanche et des anomalies de développement dans les cortex cérébral et cérébelleux. Ces cas different des cas Japonais de myopathie (Fukuyama) par l'importance des modifications de la substance blanche cérébrale et des cas de Santavuori par l'absence d'anomalies oculaires et d'hydrocéphalie. Die Gescwister litten offenbar an einer milden Form der Myopathie mit einer progressiven Hirnerkrankung, die Hirnbiopsie des einen Geschwisterkindes ergab eine Astrozytenproliferation weßen Substanz. Bei dem Patienten mit schwerer Muskelerkrankung ergab die Autopsie eine ausgedehnt fleckförmige Demyelinisation der weißen Substanz und Entwicklungsstörungen im cerebralen und cerebellären Cortex. Diese Patienten unterscheiden sich von den japanischen Fällen (Fukujama) der CMD durch den Schweregrad der Veränderungen in der weißen Substanz des Gehirns und von den Fällen Santavuoris durch das Fehlen von Augenmißbildungen und Hydrocephalus. Die einmalige Nosologie wird diskutiert.RESUMENAlteración del sistema nervioso central en las distrofias musculares congénitasSe discuten los casos de tres niños, dos hermanos y otro sin relación familiar, con distrofia muscular congénita, con afectación del Sistema nervioso central (SNC). Los hermanos padecian una miopatía relativamente moderada con enfermedad cerebral progresiva, en la que en un caso, una biopsia cerebral mostró una proliferación astrocítica en la substancia blanca. En el paciente con enfermedad muscular grave, la autopsia mostró una diesmielinización en placas en la substancia blanca, y anomalías del desarrollo en el cortex cerebral y cerebelo.
