Some of the better characterized proteins (markers) of the nervous system are described. The availability of specific antibodies to these markers has allowed the localization and assay of the proteins in tissue and biological fluids. There is some evidence that autosensitization may occur. Clinical application of these markers includes the evaluation and prognostic significance after stroke and head injury. The diagnostic histopathological use of the markers in the investigation of various tumours is summarized.

Biopsies from 60 patients with dyspepsia and endoscopically abnormal first part of duodenum were examined. The main endoscopic findings were duodenitis (35 cases), chronic ulceration (eight) and abnormal mucosal patterns (17). The latter included mucosal atrophy, thickening, irregularity, nodularity and polypoid formation. The main histological findings were duodenitis (40), gastric metaplasia (42) and gastric heterotopia (seven). The first two were commonly seen together. Heterotopia was not associated with inflammation. Good correlation existed between endoscopic and histological findings.

Staining for melanin at the ultrastructural level may act as an important diagnostic aid by helping to identify apparently amelanotic melanomas. A modified Warthin‐Starry (WS) procedure for melanin has therefore been adapted for direct application to epoxy sections. Minute amounts of melanin can thus be demonstrated in individual melanosomes, indicating a high sensitivity. Using the usual types of fixation, specificity for melanin at the ultrastructural level is high. Primary osmium tetroxide (OsO4) fixation probably induces false positive staining of lipofuscin and should not be used prior to the WS procedure, but it does not influence the results of the procedure when employed as a post‐fixative. It is not improbable, however, that the positive staining of lipofuscin and also of mast cell granules is due to the presence of melanin in these structures. The clinico‐pathological, immunohistochemical and ultrastructural features of eight cases of sinus histiocytosis with massive lymphadenopathy (SHML) recorded in Rwanda from 1975 to 1980 are reported.The main histopathological features were a massive enlargement of the lymph node sinuses which were filled with large histiocytes, a great number of which phagocytosed blood cells, especially lymphocytes, and an important proliferation of plasma cells in the medullary cords. The pathogenesis of this syndrome is discussed, particularly the role of immunological disorders challenged by a chronic, but non‐specific, infectious state. The ultrastructural study revealed no micro‐organisms and no Langerhans’ granules. Immunohistochemical staining revealed two interesting features: the presence of immunoglobulins in the cytoplasm of the histiocytes indicating the presence of an immune phagocytosis and the absence of lysozyme and α1‐antichymotrypsin (α1‐AC) from the histiocytes. Immunoperoxidase staining using five antisera to human carcinoembryonic antigen (CEA), including a mouse monoclonal antibody, was performed to investigate the expression of CEA reactivity in ten cases of granular cell myoblastoma. The granular cells were negative with four of the antisera although control sections of CEA producing colon carcinoma were positive. The single positive antiserum gave intense granular cytoplasmic staining of all tumour cells in the ten specimens studied. This reactivity was abolished after absorption of the antiserum with a perchloric acid extract of human lung to remove cross‐reacting antibodies against non‐specific cross‐reacting antigen (NCA); a procedure which did not affect the staining of colon carcinoma specimens. The results indicate that the granular cells do not contain CEA but express a related antigen and that care in the choice of primary antiserum is important if the immunocytochemical detection of this antigen is to be used as a diagnosti

The original descriptions of medullary carcinoma stressed the lack of papillary and follicular differentiation and the presence of amyloid. During the last few years, the spectrum of histological features seen in tumours of C cell origin has broadened, and papillary and anaplastic forms have been reported. We have recently studied three cases of medullary carcinoma with a major ‘tubular’ or ‘follicular’ component, giving a microscopic picture suggestive of follicular carcinoma of the thyroid. These cases were studied with routine techniques, electron microscopy, and immunohistological methods using antibodies to calcitonin and thyroglobulin. In all three cases the calcitonin localization was positive, and apart from included peripheral thyroid follicles, the thyroglobulin was negativeeven in the tumour with large follicles containing PAS positive colloid. In the cases with tubular lumina, electron microscopy showed a concentration of granules in the apical portion of the cells, with corresponding immunolocalization of calcitonin in a ring about the lumen. We conclude that medullary carcinoma should be considered in the differential diagnosis of every unusual thyroid carcinoma, even those showing glandular differentiation and containing apparent follicles, and that immunolocalization techniques using calcitonin and thyroglobulin are essential for accurate diagnosis of thyroid car

A case of hepatocellular carcinoma in a non‐cirrhotic liver of a 51–year‐old female is described. A case of primary malignant mesenchymal tumour of the liver occurring in an 86‐year‐old woman is described. This very uncommon tumour has previously only been described in children and young adults, the previous oldest being 28 years of age. The tumour was large, rapidly growing though well circumscribed and extensively necrotic. Microscopically it was mostly composed of spindle cell sarcoma without differentiating features. Epithelial lined ductules were seen throughout the tumour and degenerate hepatocytes were enveloped in the tumour peripherally.
