With the increase in the use of bone marrow trephines for diagnosis have come numerous reports that traditional methods of preparation (by decalcification and embedding in paraffin wax) should be replaced by plastic embedding to avoid decalcification. It has been argued that only by this means can the high quality preparations needed for accurate haematopathological diagnosis be achieved. The present study challenges this viewpoint and argues that with a little extra care and attention conventional paraffin embedding techniques can give equally high quality preparations. Sections prepared in this way meet the diagnostic needs of the haematologist, without requiring a separate technique to be established in the pathology laboratory solely for bone marrow trephines.

This report describes 16 cases of candidiasis, seven of aspergillosis, four of mucormycosis, two of cryptococcosis and one of nocardiosis in which the kidney was involved. Characteristic patterns of renal tissue injury were seen in each of these infections. Candida and nocardia caused microabscesses in the cortex and medulla. Tissue destruction was minimal with candidiasis and when present was confined to the papillary tips and produced only mild impairment of renal function. Aspergillus and mucor invaded blood vessels resulting in thrombosis and extensive necrosis of cortical and medullary tissue and most patients had clinical and biochemical evidence of renal disease. Cryptococcosis caused no tissue reaction or clinically evident renal disease in the two immunosuppressed patients studied.

Eighteen cases of cellular schwannoma, a recently delineated benign entity, are described. These tumours present most often in middle‐aged adults of either sex and show a predilection for the paravertebral region of the mediastinum and retroperitoneum. More than half the tumours macroscopically arose from a nerve but none was associated with von Recklinghausen's disease. The light microscopic features are remarkably consistent but are not immediately recognizable as neural in type. Means of distinction from a malignant nerve sheath tumour and from a variety of other soft tissue tumours are discussed. Thirteen cases of angiosarcoma of the face and scalp have been examined using immunohistochemistry and electron microscopy. Endothelial cell markers have been employed in an immunoperoxidase technique on tissue that has either been routinely processed, periodate‐lysine paraformaldehyde fixed (PLP) and cold processed, or fixed in methacarn. A consistent pattern of endothelial cell labelling was only achieved in the PLP fixed tissue. In this fixative the angiosarcomas were factor VIII related antigen negative,Ulex europaeuslectin positive, laminin positive, unlabelled by the monoclonal antibody PAL‐E, and positively labelled by the monoclonal antibody EN4. Ultrastructural examination of four cases showed evidence of vascular lumina in all tumours. Weibel Palade bodies were seen in only one case but three tumours showed some evidence of tight junction formation and marginal folding. This study was undertaken to assess the prevalence and characteristic hormonal profile of endocrine cells in Barrett's mucosa and to determine to what extent this profile was shared by endocrine cells of adenocarcinomas arising therefrom. In addition, lower oesophageal carcinomas, not associated with columnar metaplasia, were examined to see if they exhibited a different hormonal profile. The patients studied comprised 43 who had had multiple oesophageal biopsies, 35 who had had oesophagogastric resection for adenocarcinoma arising in Barrett's mucosa and 26 in whom the resection showed no metaplastic epithelium adjacent to tumour. Argyrophil cells were present in 90% of biopsies and resections of Barrett's mucosa combined, irrespective of the histological type of metaplastic epithelium. By immunocytochemistry the most frequently identified substance in mucosal endocrine cells was serotonin (82%) followed by somatostatin (54%), secretin (22%) and pancreatic polypeptide (17%). Gastrin, bombesin, cholecystokinin, ACTH and substance P were not identified in metaplastic mucosa in any case. The difference in expression of serotonin by endocrine cells of tumours arising in Barrett's mucosa (31%) and those not (3.8%) was statistically significant (P<0.0186). Carcinoembryonic antigen (CEA) was demonstrated in 60% of oesophageal carcinomas, both endocrine positive and endocrine negative. Focal CEA expression was seen in 4.6% of biopsies and 14% of Barrett's mucosa adjacent to tumour. These results indicate a higher prevalence of endocrine cells in Barrett's mucosa than hitherto documented and suggest that serotonin may be a useful marker in distinguishing between primary oesophageal and putative gastric cancers at the gastro‐oesophageal junction. In this study we examined 198 sarcomas, 38 carcinomas, 13 ‘tumours with a spindle cell component’and 22 malignant melanomas with a commercial monoclonal vimentin antibody. All histopathological material was formalin fixed and paraffin embedded. The results show this antibody to be a sensitive and specific marker of mesenchymal derivation or differentiation. This morphological study of small ducts within situbreast carcinoma demonstrates the changes that occur in the adjacent stroma when it stains metachromatically with toluidine blue. They are indistinguishable from those reported in areas with infiltrative tumour growth, and are accompanied by progressive damage to the duct wall. The latter may be destroyed as a result of this reaction that starts in the stroma, the tumour cells being thus provided with a possible means of escape from theirin situlocalization. This interpretation is in contrast to accepted teaching that the factors that facilitate infiltration are provided by the tumour cells. The observation is also of potential diagnostic interest.

Major histocompatibility complex (MHC) class I antigenic expression was examined on epithelial cell surfaces in normal oral mucosa, non‐specific oral keratoses. dysplastic epithelium adjacent to carcinomas and in invasive tumour islands of squamous cell carcinomas, using antibodies to HLA‐A,B,C shared determinant antigen and β2microglobulin (β2m). HLA‐A,B,C antigens were present in the basal and lower spinous cells in normal and dysplastic epithelium and in the non‐specific keratoses, but in a minority of carcinomas staining was disorganized and absent at the periphery of tumour islands. β2m staining was present in the basal and lower spinous epithelial cells in all tissues; staining was lost progressively with increasing dilution of the primary antibody until it was completely lost in the invasive carcinoma tissue at 1:400, in dysplastic epithelium at 1:800 and in non‐specific keratoses at 1:3200; in contrast, staining persisted in normal tissues at>1:3200 anti‐β2m dilution. Loss of β2m staining in the dysplastic epithelial tissues correlated broadly with the degree of cellular atypia. Cryostat sections are extensively used in the study of lymphoid tissues because the majority of antigens do not survive routine fixation and processing. A major disadvantage of cryostat section immunohistochemistry is their poor morphology. The use of periodate‐lysine‐paraformaldehyde (PLP) fixation regimes for cryostat sections was evaluated and compared with conventional acetone immersion. This fixation gives excellent morphology but poor immunostaining. The quality of immunostaining with a panel of 31 antibodies was good with brief acetone fixation followed by PLP fixation. A detailed immunocytochemical study has been carried out on a case of ductal adenoma of the breast. The lesion was oestrogen receptor positive and contained focal areas of apocrine metaplasia and marked nuclear atypia. The tumour had a pseudo‐infiltrative growth pattern and showed good evidence of both epithelial and myoepithelial differentiation.
