Immunohistological and morphometric techniques were used to study the skin after marrow transplantation with particular reference to the relationship of marrow purging, the presence of a clinical rash and histological changes to leucocyte numbers and phenotype. Recipients of T‐cell‐depleted marrow showed significant reductions in CD2+, CD4+ and CD8+ T‐lymphocytes in the first 22 d after transplantation but not after this time. T‐cell numbers in recipients of unpurged marrow were similar to those of normal donors, indicating a rapid repopulation by cells from the graft. Langerhans cells (CD1+ dendritic cells) and macrophages, on the other hand, were present in similar numbers in both groups of patients within the first 22 d; the former in low and the latter in normal numbers. Biopsies exhibiting graftversushost disease showed increases in CD2+, CD4+ and CD8+ T‐lymphocytes with significant lowering of the CD4: CD8 ratio. A proportion expressed markers of activation and HNK1+ cells and macrophages were also increased. Biopsies exhibiting epidermal basal abnormalities only (changes identical to graftversushost disease but without detectable leucocyte infiltration on conventional microscopy) showed a minor increase in macrophages and HNK1+ cells but no other leucocyte alterations to suggest a pathogenetic link with graftversushost disease. Langerhans cells were reduced in these biopsies, however, when taken more than 22 d post‐transplant, suggesting that the epidermal changes are associated with Langerhans cell damage or repopulation. We were unable to identify any significant alteration in leucocytes in patients with strong clinical evidence of graftversushost disease but with histologically unremarkable biopsies. Five ependymoblastomas were studied by means of routine histological techniques, immunohistology and electron microscopy. The tumours were characterized histologically by medium sized, poorly differentiated cells with round or oval nuclei frequently in mitosis and by ependymoblastic rosettes. Reactions for cytokeratin and neurofilament were negative and tubular material positive for glial fibrillary acidic protein was scanty. All five tumours demonstrated positivity for vimentin and S‐100 protein. Electron microscopy showed poorly differentiated cells with high nucleo‐cytoplasmic ratio and scanty cytoplasmic organelles. Sparse rosettes were present and the cells were united by junctional complexes. Frequent rudimentary or incomplete cilia, a few basal bodies and a few short intercellular glial‐like filaments were seen. The nature of the stromal cells in formalin‐fixed paraffin‐embedded material from 23 cerebellar haemangioblastomas was investigated using antisera to intermediate filaments (glial fibrillary acidic protein, vimentin and desmin), histiocytic markers (α1‐antitrypsin, α1‐antichymotrypsin and lysozyme), glycolytic enzymes (alpha and gamma enolase and aldolase C4) and the endothelial markers, factor VIII related antigen andUlex europaeusI lectin. Most stromal cells stained positively for vimentin and the glycolytic enzymes. Occasional process‐bearing cells within the stroma stained strongly for glial fibrillary acidic protein, α1‐antitrypsin and α1‐antichymotrypsin. No stromal cell staining for desmin, lysozyme or the endothelial markers was observed, although the latter stained the vascular endothelium within all neoplasms. The findings do not support previous suggestions of an endothelial or histiocytic origin for the stromal cells. A lung biopsy specimen from a young woman with the clinical features of primary pulmonary hypertension showed grade 2 plexogenic pulmonary arteriopathy. Electron microscopy revealed ‘dark’, electron‐dense smooth muscle cells in the inner part of the media of muscular pulmonary arteries. Using a panel of five monoclonal anti‐transferrin receptor antibodies, we investigated the transferrin receptor expression in 34 human hepatocellular carcinomas of Belgian (n=6), Italian (n=7) and South African (n=21) origin. For comparison the tumours were also stained with the monoclonal antibody BK 19.9, recognizing an antigen biochemically similar to the transferrin receptor, and with a monoclonal antibody against the epidermal growth factor receptor. Hepatocellular carcinomas express large amounts of transferrin receptors as demonstrated by the intense transferrin receptor immunostaining in 33/34 cases. Differences in staining pattern between and within the tumours were not related to the degree of tumour differentiation, nor to the origin or race of the patient. In 15 cases which included non‐tumoural tissue, the tumour was more intensely stained than the surrounding liver parenchyma. The BK 19.9 immunoreactivity was generally weaker and mainly involved stromal cells, except in three cases where an intense staining of the tumour cells was seen. The epidermal growth factor receptor staining was also weaker and only in four cases was the immunoreactivity of the tumour stronger than the surrounding parenchyma. Demonstration of the transferrin receptor may be useful for the detection of malignant foci in liver biopsies. 摘要:
Fibrous hamartoma of infancy is an uncommon lesion, the histogenesis and biological nature of which are uncertain. Ten cases have been studied by light microscopy, mucin histochemistry and immunohistochemistry. The typical histological features are presented. The presence of hyaluronic acid and of chondroitin‐4‐ and ‐6‐sulphate and keratan sulphate has been demonstrated in different components of the tumour. Vimentin positivity was noted in the undifferentiated and fibroblastic components. The implications of these findings are discussed. Whether the lesion merits the designation of hamartoma or is, in fact, a benign neoplasm remains

Proteoglycans were localized using cuprolinic blue and visualized by electron microscopy in sclera from the eye of a 58‐year‐old woman with necrotizing scleritis. Samples of sclera taken from clinically normal anterior and posterior sclera and from sites involved in anterior necrotizing scleritis with ulceration and posterior scleritis showed markedly different patterns of proteoglycan distribution. Proteoglycans appeared as stain‐positive filaments associated regularly with the ‘d/e’ bands of the collagen fibrils in normal anterior sclera, but were absent from intact collagen fibrils in the zone of anterior ulcerative scleritis. In posterior sclera including a site of posterior scleritis, proteoglycans were reduced in number with more heterogeneous associations with collagen fibrils. Depletion of scleral proteoglycans therefore precedes the degradation of scleral collagen in necrotizing scleritis, which may also involve the synthesis of atypical proteoglycan types in pathologic

Two cases of basosquamous cell carcinoma of the skin with lymph node, lung and bone metastases are reported. Metastases occurred 4 and 7 years after identification of the primary tumour. Both the primary and metastatic lesions had areas of typical basal cell carcinoma and squamous cell carcinoma and also intermediate carcinomatous tissue. In the bone metastasis of one case there were rudimentary hair follicles and areas of matrical differentiation. These cases further support the existence of basosquamous cell carcinoma and emphasize its metastatic potential.
