摘要:
The effect of repeated administration of decamethonium (0.5 mg/kg) was studied by measuring adduction of the thumb in response to stimulation of the ulnar nerve at the wrist in 21 subjects under nitrous oxide–halothane anesthesia. Repeat doses were given when twitch tension had recovered to 75 per cent of control value. Four successive doses of decamethonium reduced twitch tension to 7, 35, 50 and 45 per cent of control value, and maximum tetanic tension to 3.5, 19, 29 and 21 per cent of control value. Neuromuscular transmission always increased when edrophonium (20 mg) or neostigmine (1 mg) was given after two or more doses of decamethonium, but not when edrophonium was given after one dose of decamethonium.

The effects of clinical doses of scopolamine on airway resistances of ten volunteers wete measured using a body plethysmography effects on respiratory deadspace were measured using a nitrogen meter single-breath test. In the control period, airway resistance measured during panting near functional residual capacity averaged 0.76 cm H2O/1/sec; mean volume of the respiratory deadspace following full inspiration was 0.213 I Resistance fell to 0.62 cm H:O/1/sec at ten minutes, 0.50 at 20 minutes, 0.49 at 30 minutes, and 0.45 at 40 minutes following intramuscular injection of scopolamine hydrobromide, 0.0066 mg/kg. Sixty minutes, 120 minutes, and 180 minutes after injection there were no further changes in airway resistance, the average being 0.51 cm H2O/I/sec Respiratory deadspace was 0.285 I at 59 minutes, 0–240 at 120 minutes, and 0.228 at 180 minutes after injection. When scopolamine hydrobromide was administered to four subjects in doses of 0.0022, 0.0044, and O.0066 mg/kg the change in airway resistance was greatest at the intermediate dose. This was consistent in each subject at each of the three levels of lung inflation at which airway resistance was measured. In contrast, respiratory deadspace increased progressively with each increase in dose. These results are comparable to those obtained previously in response to injection of atropine.

摘要:
Effects of intravenously injected Idocaine (5 to 25 mg/kg) on synaptic transmission were studied in decerebrate or spinal cats by stimulating a dorsal lumbar root and recording the reflex action potential evoked in the ventral root. Lidocaine increased the amplitude of the monosynaptic spike two- to 25-fold, and reduced the polysynaptic response by 10 to 50 per cent of the control value. Facilitation of synaptic transmission presumably resulted from selective depression of segmental inhibitory synaptic influences and appeared unrelated to blood pressure. The convulsant action of Idocaine thus may be due to preferential depression of inhibitory control normally exerted over excitatory input

Glucose uptake, lactate formation and glycogen synthesis by isolated rat hemidiaphragm were measured in the presence and absence of diethyl ether and insulin. Ether caused a significant decrease in glucose uptake by isolated skeletal muscle. The increase in glucose uptake which was induced by insulin was not blocked by ether. Ether caused a significant increase in the rate of lactate formation by isolated skeletal muscle, and added insulin doubled this effect of ether. Ether induced glycogenolysis in the isolated rat hemidiaphragm and also altered the disposition of the increased glucose uptake by the tissue which occurred in the presence of insulin.

sixteen operations for pheochromocytoma have been performed in 14 patients since 1964. Phenoxybenzamine was administered orally before 12 operations. This therapy ameliorated symptoms, brought the blood pressure under control, and expanded plasma volume. Blood pressure increases still occurred during manipulation of the tumor, and supplemental phentolamine was required in most patients. Methoxyflurane was used in 12 operations, halothane in three, and the combination of N2O-meperidine-succinyl-choline in one. None of the patients anesthetized with methoxyflurane (11 also received phenoxybenzamine) developed cardiac arrhythmias during surgery, although each of the other four showed frequent premature ventricular contractions and one had ventricular tachycardia. Hypotension after removal of the tumor was treated by expansion of the blood volume and brief use of a sympathomimetic amine in some cases; only one phenoxybenzamine-treated patient was givenl-norepinephrine for longer than 15 minutes. The findings indicate that the combination of phenoxybenzamine pretreatment and methoxyflurane anesthesia is useful in patients with pheochromocytoma; patients so treated have a relatively stable blood pressure course and a low incidence of cardiac arrhythmias during surgery.
