Certain pathologic conditions related to pregnancy have been associated with biochemical and clinical hyperthyroidism. These conditions include trophoblastic tumors (molar gestation and choriocarcinoma) and hyperemesis gravidarum. More recently, it has been recognized that multiple gestation and some otherwise normal-appearing singleton gestations will result in hyperthyroidism. In this article the authors review the natural history and clinical significance of these conditions and conclude with an analysis of the lines of evidence that suggest that the cause of the hyperthyroidism is human chorionic gonadotropin secreted by the trophoblast.

Vulvo-vaginal inflammation is the most common gynecologic complaint in the prepubertal girl. The vast majority of cases of vulvo-vaginitis in the pediatric population are cared for by pediatricians, family practitioners, and pediatric nurse practitioners. Although most obstetrician/gynecologists are quite experienced in caring for vaginitis in women who are in their reproductive years, they often have limited exposure during their training to the evaluation and management of the child with vulvo-vaginitis. Gynecologists are likely to be consulted in those cases in which the child is not responding to therapy. Therefore, it is important, for the clinician to have a standard approach to evaluation of the child with vulvo-vaginitis to optimize therapy and to identify children suffering from sexually transmitted infections requiring examination by child protective services.
