Reducing the rate of preterm delivery in twins is a major goal of obstetricians and other health care providers worldwide.1,2This article discusses three important aspects of issues relating to preterm delivery rates: 1) the frequency of preterm births (compared with singleton pregnancies) and its consequences in terms of adverse outcomes, 2) various techniques proposed to prevent preterm births among twins and their limitations, and 3) the concept of the "Twin Clinic" as a means of providing a higher level of care to these high risk pregnancies and reducing their rates of preterm delivery.

The incidence of higher order multiple pregnancies has increased during the past 20 years, mainly due to the increasing availability of ovulation-inducing agents and secondarily to use of techniques of assisted reproduction. Parallel with this, the incidence of cesarean deliveries increased substantially in developed countries. The popularity of abdominal delivery resulted partly from the somewhat dogmatic belief that cesarean section may relieve intrapartum fetal distress and thereby improve perinatal outcome. Not surprisingly, the question was raised as to whether cesarean section would improve the historically poor neonatal outcome of triplets. This concern was the origin of the extensive use of cesarean section in higher order multiple births. Recent data have suggested, however, that a less invasive approach might be safer and more effective.

Until recently, estimating the frequency of loss of twin gestations has been difficult, partly because of the fact that, until sensitive human chorionic gonadotropin (hCG) assays were developed, the loss rate before the first missed menses was speculative. Moreover, any calculation of the total rate of pregnancy loss in twins must include not only complete pregnancy loss, but also spontaneous resorption of one twin. The frequency of the latter circumstance, popularly known as the "vanishing twin" phenomenon, only has become quantifiable in recent years after the development of ultrasound. In this article, we will review contemporary studies that allow a determination of the total rate of loss of twin pregnancy through the first two trimesters. In addition, we will consider possible etiologies of loss and methods to improve survival.
