摘要:
This article discusses the common health problems of adolescents with an emphasis on the differences between those affecting girls and boys. Statistics are given for mortality, hospitalization, and out-patient health visits from several different sources. Accidents and violence are the leading causes of death in adolescents but they affect boys much more often than girls. Teenaged girls are frequently hospitalized for health problems related to sexual behavior, mainly pregnancy and delivery related care and sexually transmitted diseases. Emotional and psychosomatic disorders are very common among adolescents of both sexes. Three general considerations important for the provision of health care to adolescents are discussed in detail. These include privacy and confidentiality, prevention of adverse health consequences of risk-taking behaviors, and the utilization of professionals from several different health care disciplines.

This paper emphasizes the need for adequate nutrition in order to sustain normal growth in adolescence. The special needs of the adolescent athlete and the pregnant teenager are also reviewed. Adolescent athletes require added nutrient intake for optimal performance. And, if pregnancy occurs, it imposes severe nutritional demands on the adolescent which must be met to prevent low birthweight infants. Although many teenagers consume a good deal of their food outside of the home, there seems to be little evidence of widespread nutritional deficiencies except with respect to iron. However, there is a high incidence of obesity and anorexia among female adolescents and these disorders are discussed.

Because an attractive appearance is so essential to the feminine gender role, the search for beauty causes special adjustment problems for adolescent girls. Psychologically they suffer from negative body image, lowered self-esteem, and achievement conflicts. Physically their health is undermined by current beauty norms which foster eating disorders, cosmetic acne, and breast surgery. The effects of physical fitness programs, the role of the media, and the influence of changing gender roles are also discussed.

The first pelvic examination is a rite of passage into American womanhood. Indicators and special considerations for performing a pelvic examination for a teenaged female are reviewed. Factors in the history, physical examination, and laboratory evaluation of the adolescent are presented. The sequence of pubertal events before, during, and after menarche is discussed so that menstrual disorders are presented in the context of normal development. The author presents a synthesis of her approach to caring for adolescents with a review of current literature and resources available to health professionals.

Adolescent pregnancy represents a triple developmental crisis to young women, that of adolescence, pregnancy, and establishing a relationship with a member of the opposite sex. Physiologically the adjustments of pregnancy are superimposed on those of pubescence. Psychologically and intellectually the pregnant adolescent is still developing, and it is thus difficult for her to meet the physiological demands of pregnancy. Society has responded by developing comprehensive programs for intensive care which have reduced considerably the risks of pregnancy. Contraceptive knowledge has not totally prevented the problems of teenage pregnancy, in pan because the adolescents who become pregnant are not sufficiently developed intellectually to perceive that today's actions may prevent future consequences. This paper reviews the literature and synthesizes the published studies with personal experience gained in over five years of professional work with teenage women.

The most common form of scoliosis (lateral curvature of the spine) is adolescent idiopathic scoliosis (AIS). The prevalence of AIS is 10-20 individuals per 1,000 population screened. Progressive AIS affects females more frequently than males. Only 1-2 per 1 ,000 individuals have progressive AIS and need brace or surgical treatment. Early screening programs for scoliosis and other spinal deformities are instituted in schools in the fifth to ninth grades to achieve early detection of progressive AIS. If detected early, progressive AIS can in most cases be successfully treated by use of an external spinal brace. More severe scoliosis is successfully treated with surgical instrumentation and spinal fusion. Although much progress has been made in understanding the natural history of AIS, its cause remains unknown.

Cigarette smoking rates among teenage females have risen progressively since 1968 until surpassing teenage males in 1979. Psychosocial factors underlying the initiation of smoking include peer pressure, adult role modeling and prosmoking messages in advertising, with smoking potentially representing a desired set of personality characteristics. Regular smoking leads to the development of a dependence process, with cessation often difficult to achieve and maintain. Preventing smoking onset is a responsibility of health professionals and educators. Social psychological approaches teaching social skills and techniques for resisting smoking have been effective in reducing rates of initiation in JHS experimental programs.
