AbstractThe accuracy of standard clinical and radiologic methods in detecting invasion of the laryngeal framework or the presence of a transglottic tumor was examined by comparing clinical and pathologic findings in a series of 50 randomly selected laryngectomies for squamous carcinoma. Forty‐two of the patients had received radiation therapy as primary treatment. The presence of pain referred to the ear immediately before laryngectomy indicated invasion of the laryngeal framework or spread into the extralaryngeal soft tissues in 11 of 12 patients. Transglottic tumors were correctly identified in 10 of 14 patients from laryngeal tomograms, but seven additional tumors were incorrectly designated as transglottic. Both transglottic tumors and infraglottic extension were seriously underdiagnosed by direct laryngoscopy. Framework invasion was seen to be a feature of transglottic tumors 2 cm or more in diameter. AbstractSuprahyoid lymph node dissection often is used as part of the surgical treatment of cancers of the anterior two thirds of the tongue and the floor of the mouth. In recent years the use of this procedure has become less widespread, but it is still advocated by some surgeons as a sound operation in the treatment of cancer. Anatomic, roentgenographic, and pathologic evidence is presented to show that this may not be the case. The evidence points out that such neoplasms do not metastasize in an orderly or predictable pattern. These patients may have isolated metastases in any region of the neck. If the decision is made to treat the neck in these situations, the whole neck should be treated. We feel that suprahyoid neck dissection has a very limited role to play in the management of cancers of the anterior two thirds of the tongue and the floor of the mouth and should be employed only when it will be of help in developing an adequate margin around the primary lesion.

AbstractThe advent of both improved imaging systems and new radioactive agents has increased the effectiveness of nuclear medicine in diagnosing and treating diseases of the head and neck. In this second of two articles, we discuss radionuclide bone imaging and the role of nuclear medicine in the management of thyroid disease. Radionuclide bone imaging is useful in the differential diagnosis of sinusitis, the early detection of head and neck fracture, the assessment of temporomandibular joint disease, and the identification of local extension of primary head and neck carcinoma. In the management of thyroid disease, radionuclide technology is uniquely helpful in the diagnostic evaluation of the thyroid nodule and radioactive iodine continues to play a major role in thyroid cancer therapy.

AbstractThe major advances in radiation oncology in this century was the development of megavoltage radiation in the 1950s, which greatly expanded the clinical utility of ionizing radiation in the treatment of many forms of cancer, including cancers of the head and neck. The combination of radiation and surgery for improved control of local and regional cancers followed. The use of interstitial implant therapy is growing rapidly because of new materials and techniques, thus expanding the radio‐therapeutic options available to the oncologist. Investigations into several other therapeutic modalities currently are being carried out. These modalities include the use of particle radiation, oxygen‐mimicking drugs, and hyperthermia, primarily in efforts to overcome the need for the oxygen effect in tumors. AbstractA review of the recent literature on surgery of the facial nerve is presented. Developments in the treatment of hemifacial spasm, nerve grafting, reanimation surgery, and diagnostic testing are described. Reports of facial paralysis following mandibular fractures are included. The roles of brain stem evoked response (BSER) audiometry, electroneuronography, and CT scanning in the evaluation of facial nerve disorders are noted.
