Seven patients have been operated on at St Vincent's Hospital since 1954 in attempts to relieve superior vena caval obstruction. Six of them had carcinoma of the lung, and synthetic bypass grafting was performed in four. The seventh patient had a malignant thymoma excised, with resection of the superior vena cava and replacement by a free pericardial graft. The intraatrial baffle used in physiological repair of transposition of the great arteries can fibrose and alter its shape, producing venous obstruction, so that refashioning may become necessary as a secondary procedure. Persistence of a left superior vena cava with unroofing of the coronary sinus appears to be due to a single developmental complex. The incidence and ætiology of hepatoma in Papua New Guinea are discussed. A review has been made of 43 cases of hepatoma diagnosed at the Port Moresby General Hospital between 1960 and 1970 and of 24 cases diagnosed in 1970 and 1971. The surgery of hepatoma is discussed. Three cases are reported of proven parathyroid adenoma located in a lobe of the thyroid. This situation must always be borne in mind when searching for the parathyroid in the neck, and the excision of a lobe of the thyroid may save the patient from an unnecessary sternotomy and mediastinal exploration.

The experience of renal trauma at the Royal Children's Hospital, Melbourne, is analysed. In assessing the 78 children involved, Nunn's (1962) classification into contusion (Type 1), cortical lacerations (Type 2), calyccal lacerations (Type 3) and complete tears (Type 4) was extended to include a fifth category, where the main damage was to the renal vessels. The clinical features and the radiological and operative findings were used to classify the lesions. Palls and motor vehicle accidents were the most common causes of injury. The examination of the methods of management indicates that in Types 1 and 2 non‐operative methods are usually sufficient; in Type 3 initial non‐operative management is indicated, but long‐term follow up is essential; in Type 4 injuries and vascular lesions the need for early diagnosis is paramount, and most patients will fare best with early surgical intervention.
