If nurse practitioners are to achieve their goal of serving as the principal providers of primary health care and having a major voice in health policy decisons, they must increase their political effectiveness. Of all the problems that nurse practitioners face today, this is one of the most serious. This article identifies some of the reasons why nurse practitioners have failed to achieve maximum political effectiveness, such as lack of national stancardization of the role, titling confusion, and the lack of research to demonstrate that the nurse practitioner is the most appropriate and cost effective primary health care provider. Next, the consequences of the above are explored. Finally, some solutions are proposed, the most important of which is the establishment of a strong national nurse practitioner organization to serve as the voice of all nurse practitioners.

This study determined differences in delivery outcomes of primiparous women (n = 107) by certified nurse midwives versus similar deliveries by experienced obstetricians through a retrospective chart review of low risk births. The use of both forceps and midline episiotomies were found to be significantly higher in the obstetrician group. Obstetricians also were more likely to use anesthesia than the midwife group. No significant differences were found in the number or degree of perineal lacerations or in the mean Apgar scores.

Twenty‐five percent of the adult population needs treatment to reduce elevated cholesterol levels. A national educational effort spearheaded by the National Cholesterol Education Program requires the support and active participation of health professionals, especially nurses and nurse practitioners, if it is to be effective. The NCEP Adult Treatment Panel recently established guidelines for treating high blood cholesterol in adults. The therapeutic relationship, the working bond between the nurse practitioner and patient, is considered to be a significant factor that affects treatment outcome. Components of the therapeutic relationship viewed from the nurse practitioner's perspective and also from the patient's viewpoint are reviewed. Modes of communication the client uses directly influences the therapeutic relationship or alliance. Two communication modes are used to exemplify two case examples in which the patient's mode of perceiving his or her world is an important consideration in the working alliance.

Urinary incontinence is a major health problem affecting the lives of many elderly people. A formal urologic examination often is not required for proper diagnosis and successful treatment of incontinence. This article focuses on safe, accurate, and inexpensive methods of incontinence assessment that can be performed in a variety of settings by nurse practitioners. The evaluation described incorporates medical, behavioral, and physiologic assessment of deficits commonly encountered in older incontinent individuals.
