摘要:
ABSTRACTHealth care costs are a major issue in the health care system today. One suggested method of containing costs is the efficient use of presently available facilities and health care personnel. Categories of hospital charges included in a normal hospital maternity stay are described. The mean hospital bill for CNM patients was $114 less than the mean hospital bill for the physician provider patients. These findings may indicate that the use of CNMs as primary care providers could reduce hospital maternity costs while meeting the consumer demand for safe, satisfying, and economical care.

ABSTRACTThe application of water externally to the body for therapeutic effects is a practice called hydrotherapy which dates back to ancient times. Immersing the body in water is an effective method of relieving pain, promoting muscular relaxation, and reducing psychological tension due to the hydrothermal and hydrokinetic properties of water. Using a warm bath to decrease the pain associated with labor and to enhance the process of a normal labor can be safely instituted by nurse‐midwives. The bath should feel comfortably warm, cover the body completely, and may continue for as long as the woman is comfortable. Bathing in no way compromises the fetus and thus appears to be an ideal analgesic for use during labo

ABSTRACTControversy exists concerning the role of nurse‐midwives in tertiary care centers. By tradition, preparation, and practice, nurse‐midwives have not sought to work in the areas of high‐risk obstetrics. However with the advent of modern technology and the prevailing economic and legal climate, many more women are finding themselves referred to high‐risk centers for obstetrical care. All of these factors contributed to the formation of the nurse‐midwifery service at the University of Miami, Jackson Memorial Hospital, Miami, Florida in January 1977. The service is described as it evolved over four and a half years. The success of the model led to the expansion of the philosophy and the development of the three principles of practice–independent management, comanagement, and collaborative management.
