A retrospective study of 990 ovarian carcinomas treated at our hospital during the years 1945 through 1964 is presented. The study comprises serous, mucinous, endometrioid, undifferentiated and mesonephroid tumors. Histologic classification and clinical staging were performed according to the systems adopted by FIGO.Unilateral tumors had a significantly better prognosis than bilateral tumors, and the occurrence of ascites in Stage I was a bad prognostic sign. In Stages II and III, complete removal of all macroscopic tumor tissue was of great importance. For patients with potentially malignant tumors, survival curves showed a small, hut steady decline during the entire observation period up to 20 years after primary treatment, and no less than 15–25% of the patients died from cancer. Among the truly invasive carcinomas, mesonephroid tumors of clear-cell type had the best prognosis, and then endometrioid, mucinous, mesonephroid with tubular pattern, serous and undifferentiated tumors, in that order. Serous carcinomas had a great tendency to a protracted clinical course with late recurrences up to 20 years after primary treatment.

A rapid preincubated, charcoal-dextran radio-immunoassay procedure was applied to measurements of immunoreactive human chorionic gonadotropin (IR-HCG) during normal pregnancy and the results compared with those of biologic assays. Analysis of several available HCG samples of varying degrees of refinement indicates the need for an international reference preparation for radio-immunoassay. The characteristic pattern of serum IR-HCG during normal pregnancy is similar to that obtained by bioassay during the first trimester but differs significantly later in pregnancy, showing a gradual secondary rise to term.

The place of midforceps delivery in obstetrics remains a disputed one. The authors discuss their experience with 117 midforceps deliveries accounting for a midforceps delivery rate of 10%. A comparison is made between results obtained with traditional indicated and elective midforceps delivery. The authors conclude that the danger does not lie in the midforceps operation but rather in those circumstances which precede and require its use.

A twin vertex delivery was accomplished with metric divergent forceps. The forces of peak and average compression and traction are typical of a much larger series when compared for maternal parity and type of instrument used. This case report exemplifies the function of divergent forceps as a third-degree lever with sequential reduction in the ratio of compression to traction in consecutive deliveries.

The birth of a conjoined monster (teratus catadidymus) at our hospital which resulted in fetal death and maternal morbidity prompted a review of the subject of conjoined twins. The experience of 5 such cases in 20 years is reviewed and discussed with regard to diagnosis and management.

Twins totaling 569 pairs were studied in relation to type of twin placentation. Separate diamniotic-dichorionic placentas were rarely associated with monozygosity. This and the fact that monochorionic placentas are all monozygotic make examination of twin placentas extremely useful in forecasting zygosity. Relatively heavy infants as well as twins with large intrapair birth-weight differences were common in the separate diamniotic-dichorionic group, suggesting independent intrauterine growth of co-twins. The converse was true in twins with fused diamniotic-dichorionic placentas, who also had the lowest death rates. Perinatal death rate Was the same in whites (14.7%) and Negroes (14.3%), highest in male-male twin pairs, but lowest in male- female pairs in Negroes and female-female pairs in whites. Compared to singleton deaths, the frequency of congenital malformations was not higher in twin deaths, but the types of malformations found in monochorionic deaths were often multiple and lethal.

Measurements of the biparietal and anteroposterior chest diameters were made on 1079 newborn infants. These measurements were compared with fetal weight and gestational age. Estimates of birth weight from the biparietal diameter alone had a prediction error standard deviation of ± 350.36 g. When the anteroposterior chest diameter was used the prediction error standard deviation was ± 364.14 g. When both of these diameters were used, the prediction error standard deviation was 289.73 g. A similar comparison with gestational age, using both of these dimensions, was not significantly better than predicting gestational age from the last menstrual period. Formulas and graphs to predict fetal weight and gestational age from these measurements are presented.

Cervical biopsies in all patients and conizations in selected patients are indicated before irradiation and surgical therapy to detect spread of endometrial cancer to the cervix. Cervical biopsies disclosed endometrial cancer in 1.5% of patients. Conizations in certain selected high-risk patients whose cervical biopsies were negative, disclosed an additional 2.2% patients with cervical involvement. Since therapy to improve survival of patients in whom endometrial carcinoma has extended to the cervix must be radically applied, the gynecologist must exclude the presence of such cervical spread before embarking on more conventional treatment.

Eighty-one cases of chorioangioma in the Collaborative Study were analyzed, of which 5 were in twin pregnancies. In n population consisting of approximately an equal number of whites and Negroes, chorioangiomas were found more frequently in whites (58 patients) than in Negroes (23 patients), and in females (49 patients) more often than in males (37 patients). Two infants had neonatal thrombocytopenic purpura unassociated with skin hemangioma. Acute toxemia was a significant complication in both whites and Negroes. In Negroes, in particular, twin rates in current and prior pregnancies were conspicuously high, and the gravida herself was often a twin. Congenital anomalies were frequent in twins. In surviving single births, the incidences of several malformations were significantly higher than in controls. Interesting interrelationships among chorioangioma, skin hemangioma, single umbilical artery and twinning are discussed.
