The clinical features, treatment, and results in a series of 78 patients with vulvar malignancies are presented. Appropriate statistical methods are used to evaluate the effect of radical operation on survival rates. The results obtained demonstrate that more aggressive surgical management can result in significantly better cure rates than previously achieved.

For 1734 prenatal patients the indirect immunofluorescent antibody method was used to determine the presence of rubella antibody: 17% were found to have antibody titers less than 1:8 pointing to their susceptibility to rubella infection. There was a significantly greater frequency of seronegative patients among Orientals and among those who had no history of rubella infection. Patients tested for rubella antibody are provided documentation of their immunity for future reference. Rubella antibody screening during pregnancy identifies a group of seronegative women who are ideal candidates for postpartum rubella immunization.

The spiral arterioles in normal proliferative and secretory endometrium were compared with those of hyperplastic and contraceptive-affected endometriums. No smooth muscle or endothelial cell hyperplasia was present in 100 samples of proliferative and secretory endometrium. Two thirds of 100 hyperplastic endometriums showed smooth muscle hyperplasia of the spinal arterioles; one-third of the specimens also had endothelial proliferation of the vessels. The same changes were observed in one-third of 78 endometrial biopsies from patients taking oral contraceptives, both combination and sequential. Estrogen may be the causative agent in both types of altered endometriums.

A method used to study the microvasculature of the human uterus, which included the filling of the smallest vessels in the endometrium by radiopaque medium, is described. The microradiographic technic, employing Kodak Maximum Resolution plates, allowed much greater magnification of the radiographs of the small vessels than has hitherto been possible. The problems encountered in the injection and microradiography of these small vessels are discussed.

Bacteroides were found in mixed infection the uterus of 111 women who had nonfatal, incomplete, septic abortion. Half of these women had bacteroides bacteremia. Other nonfatal paraparturient mixed infections with bacteroides included amnionitis, endometritis, and wound infections after cesarean section. Five women had repeated infections.

A total of 2372 gestational females were observed for right ovarian vein syndrome. Only one patient was found to have such pathologic association which subsequently necessitated operation. The so-called right ovarian vein syndrome is probably not as prevalent as earlier reports suggest. However, as in any obstructive uropathy, one must be guided by the usual persistence of obstruction, architectural deterioration of the renal collecting system, and infection resistance in making the decision regarding surgical treatment.

Protein-bound fucose was measured in the serum of women in two conditions: (1) during and after pregnancy, and (2) before and during oral contraceptive therapy. These measurements were made during oral glucose tolerance test (6 hr for pregnant group; 3 hr for oral contraceptive group). While the glucose and insulin levels fluctuated with time, the serum fucose levels were constant. Both glucose and insulin levels were altered by the two conditions studied, but the serum fucose was not.

The effects of administration of norethindrone 1 mg with mestranol 0.08 mg to 50 nursing mothers are described. The effect on lactation was inhibitory when compared to controls, demonstrated by lower infant weight gains in spite of an increase in supplemental calories given to the infants. The duration of lactation was decreased significantly when compared to 50 controls. This effect was more pronounced in primiparas when compared to those with previous lactation success. The medication had no obvious effect on involution of infant breast tissue. No deleterious effect on maternal genital involution was noted.

Continuous daily oral administration of 1 mg of stilbestrol during the 3 months following the first injection of the contraceptive, depo-medroxy-progesterone acetate, 150 mg, failed to diminish the incidence of uterine bleeding when compared with controls. Such supplemental estrogen did not significantly alter the appearance of endometrial biopsies or karyopyknotic indices. Cyclic administration of supplemental oral estrogen for 3 weeks of a 4-week period caused regular withdrawal uterine bleeding. It was concluded that there is little advantage in giving supplemental oral estrogen to patients using long-acting injectable progestogen for contraception.

Clomiphene treatment to restore fertility in 142 anovulatory women resulted in ovulation in 72% of the patients with nonspecific anovuIation, in 56% of those with secondary amenorrhea, and in 53 % of the Stein-Leventhal group. Forty-seven pregnancies were initiated in treatment cycles (33 livebirths, 14 abortions). Twenty patients who failed to respond to clomiphene alone underwent retreatment that included HCG; 5 of 60 such treatment cycles were ovulatory and 1 patient achieved a successful pregnancy. The clinical course of 1 patient who experienced an ovarian hyperstimulation syndrome is described.
