Paracervical block using 20 ml of 0.25% bupivacaine alone, or with epinephrine 2.5 μg/ml, was given to 16 patients about to undergo elective cesarean section, who were then delivered between 5 and 30 minutes after the injection. Plasma bupivacaine concentrations were estimated in mother and baby. Bupivacaine alone produced maternal plasma concentrations double those after bupivacaine with epine-phrine. High concentrations of bupivacaine were not found in the neonates in this series. Direct intoxication of the fetus by local anesthetic agents is a possible cause of fetal brady-cardia and intrauterine death during paracervical block. Reasonable evidence is available to suggest that bupivacaine in a concentration of more than 0.25% should not be used for paracervical block in obstetrics.

In this series of 1199 women with vaginitis, errors in diagnosis due to false-positive and false-negative findings when the Papanicolaou smear was used as the criterion for diagnosing and treatingTrichomonas vaginalisinfestation reached 48.4%. The Papanicolaou smear, therefore, should not be relied upon for the diagnosis of Trichomonas vaginalis. Confirmation by smear and culture should be obtained before treatment.

During the 11-year period from 1950 to 1961, the authors treated 1787 patients with cancer of the cervix, of whom 173 (10%) had only a cervical stump. The 173 patients could be divided into two groups—those in whom the neoplasm was found within 2 years of the subtotal hysterectomy (probably representing cancer present and left behind at the time of operation), and those in whom the malignant tumor appeared more than 2 years after the uterine operation. The survival rate in the latter group was the same as that for the group with cancer of the cervix. In this series, the management of cervical stump cancer was the same as that for cervical cancer, varying with the stage of the disease.

Cervical tumors resembling adenoid cystic carcinoma (cylindroma) are rare and few have been reported. Little is known of the true biologic behavior and therapeutic responsiveness of cylindroma. This report presents a cervical tumor with features of the cylindromatous type of carcinoma. The patient shows no evidence of recurrent tumor 16 months after incomplete removal of the tumor and postoperative radiotherapy.

In a series of 125 patients, either colposcopically-directed or random biopsies were taken for histopathologic diagnosis when cytologic smears were abnormal. To date, the cervix of 60 of 74 in the directed group and 40 of 51 in the random group has been evaluated completely by conization. Approximately 90% of the group who had directed biopsies showed no significant difference in diagnosis when compared with the definite cone. In the random group, 17.5% had significantly more advanced disease on conization and 50.0% had some degree of more advanced stage of neoplasia at final diagnosis than was made at random biopsy. With strict adherence to limiting criteria for selecting patients who require cervical conization, colposcopy and directed biopsy in the remainder might effectively replace conization in almost 90% of women with abnormal smears.

A preconceptional method for the treatment of incompetent cervix, using endocervical electro-cautery, is presented. The technic is safe, simple and readily reproducible. It has yielded good results and has few undesirable features.

Fifty-seven patients undergoing spontaneous or therapeutic abortion were found to be Rh and indirect Coombs-negative. Rhogam was given to 21 patients and no sensitizations occurred. Of the 36 patients who received a placebo, 2, both primigravidas in the first trimester, were found to have a positive indirect Coombs at followup.

A generalized convulsion and consumptive coagulopathy with generalized hemorrhage occurred in a patient 21 weeks pregnant 16 hours after intra-amniotic injection of hypertonic saline and immediately after complete abortion. Serial studies of coagulation factors were consistent with disscminated intravascular coagulation. Neurologic evaluation revealed no obvious cause for the seizure. Blood volume replacement and supportive therapy were followed by total recovery, supporting the self-limited feature of this syndrome.

Several investigators have suggested a relationship between habitual abortion and infection withToxoplasma gondii. In this study patients with repeated spontaneous abortions were studied for the presence of toxoplasma antibody by means of the methylene blue dye test (DT). A group of obstetric patients without abortions were used as controls. Five of 25 patients with abortions had a positive DT, with titers from 1:16 to 1:256. Ten of 47 controls had a positive DT, with titers of 1:16 to 1:512. Thus, the group with abortions had no greater prevalence (20%) of DT antibodies than did controls (21.3%). These data suggest that in this population with habitual abortions there was no evidence to support an etiologic relationship between toxoplasmosis and fetal wastage
