A prospective study of 84 patients with gestational diabetes for whom the route of delivery was determined by the double-blind technic is presented. With strict attention to dietary management, it was possible to maintain the basal maternal plasma glucose concentrations below 100 mg/100 ml from the initiation of management until delivery. Under these circumstances, perinatal mortality and morbidity were not increased with advancing gestational age. Neonatal complications were minimal for either route of delivery. Maternal weight gain and weight at delivery were positively correlated with birthweight of the infant, yet the degree of abnormality in the glucose tolerance test had no influence on the outcome of the pregnancy. Postoperative maternal morbidity following cesarean section was of a minor nature. No advantage to preterm delivery by cesarean section was found in this study, since one of the contraindications to vaginal delivery—fetal macrosomia—was averted through maternal dietary control.

A large mixed population of pregnant women was studied by sonography in an attempt to define the relation between sonar biparietal diameter (BPD) and fetal age. In the interval of 20-29 weeks, fetal maturity can be predicted from BPD by linear regression, with a maximal deviation from the mean of 11 days in 95% of the population. The use of regression analysis, however, is not statistically feasible in estimating fetal maturity after 29 weeks. A different method is presented in which third trimester BPD values are used to assign precise probabilities to the estimates of fetal age. This approach transforms the sonar gestational age for this later period into a clinically meaningful parameter. Sonographic study of all highrisk pregnancies is recommended between 20- 25 weeks. Such early evaluation establishes fetal age and permits easier differentiation of dysmaturity if growth retardation occurs in the latter part of pregnancy.

Iron saturation of serum transferrin (Fe/ TIBC) has been found useful in assessing the iron status of women in the nonpregnant slate. Sixty-two pregnant anemic patients with various Fe/TIBC ratios were studied with respect to their response to iron therapy. Patients with Fe/TIBC below 20% responded to iron with a significant mean increase in hemoglobin and a restitution of the mean Fe/TIBC to above 20%, while patients with Fe/TIBC above 20% showed no mean increment in their hemoglobin after comparable treatment. In patients with Fe/TIBC below 20%, the TIBC showed a variable but predictable response to successful therapy dependent on pretreatment values in that posttreatment values tended toward 400 µg/100 ml. The Findings support the concept that the Fe/TIBC ratio is as useful a test for evaluating iron deficiency in the pregnant as in the nonpregnant state.

Based on human and sheep studies and a literature review, it is concluded that a variety of fetal heart rate patterns can reflect umbilical cord compression. At the same time, maternal aortal-caval compression during uterine contraction (Poseiro effect) can be associated with the typical umbilical cord compression fetal heart rate pattern. This same heart rate pattern occurs when the hypoxic sheep fetus grunts in utero. In reviewing fetal heart rate recordings and through experience with actual clamping of the umbilical cord, a heart rate pattern of abrupt deceleration with overshoot (or secondary acceleration) became apparent. This deceleration-overshoot pattern was associated with newborns requiring resuscitation and is probably caused by a fetal hypoxic insult which is rapidly relieved. Because the commonly defined cord occlusion pattern is imprecise and because the secondary acceleration (overshoot) after deceleration appears to be significant, it is proposed to redefine the “cord compression” (or intermittent fetal hypoxia) heart rale pattern.

Medical literature contains conflicting reports concerning serum levels of placental (heat stable) and other (heat labile) isoenzymes of alkaline phosphatase (AP) in normal and abnormal pregnancies. Only one investigation has been devoted extensively to the study of placental AP in normal pregnant patients near term. In this study the isoenzymes of AP present in the sera of 88 carefully selected pregnant subjects at term have been identified and quantitated using electrophoresis. The mixtures of isoenzymes identified arc studied using physicochemical inhibitors. Pregnant subjects near term have widely variable amounts of placental AP and also have larger amounts of liver AP than nonpregnant subjects. The data obtained suggest the existence of a maternal homeostatic mechanism to control blood levels of total enzyme. Physicochemical studies reveal that placental AP retains its individual characteristics when mixed in the scrum with the liver isoenzyme.

The puerperal blood disappearance rates of three hormones (unconjugated 17β estradiol, total estriol, and human chorionic somatomammotropin) and three enzymes (heat stable alkaline phosphatase, cystine aminopeptidase, and diamine oxidase) purported to be useful as indices of fetoplacental status have been simultaneously determined in 7 subjects. Disappearance rates of the hormones were faster (minutes) than the enzymes (days) providing a theoretic lime advantage of the former in minimizing time lost between fetoplacental compromise and its earliest possible detection.

A rare case of sacculation of the uterus secondary to incarceration of a retroverted gravid uterus is described. The inclusion of such a condition in the differential diagnosis of pelvic masses during pregnancy has been stressed. A review of similar cases is presented, and the management is discussed.

A case is presented in which a new chemotherapeutic agent, dibromomannitol, was used to treat a pregnant patient for chronic myelogenous leukemia from the 10th through the 17th week of gestation. At birth, and during the subsequent 4 years of followup, the patient's child has remained normal. The results obtained in this case are presented to aid other physicians in the evaluation and treatment of future cases of chronic myelogenous leukemia during a pregnancy in which the use of dibromomannitol is considered.

Tubo-ovarian abscess in the late second trimester of pregnancy is almost nonexistent. This paper presents a case in the sixth month of gestation, which terminated successfully with a live infant and mother. A review of the literature, as well as possible etiologic mechanisms, and methods of management are also presented.
