Two cultures of human adult lymphocytes and four cultures of umbilical cord blood were exposed to diagnostic ultrasound during the entire incubation period of 72 to 90 hours. The control cultures were incubated under the same condition without being exposed to ultrasound. Nine hundred twenty-six cells from test and control specimens were scored by a doubleblind method. Both test and control groups of cells showed minimal damage, but that of the test cells was not significantly greater than that of control cells.

One thousand one hundred and fifty monitored labors were studied. Endometritis was rare and postpartum febrile morbidity could not be attributed directly to the direct technics used for intrapartum monitoring. Three asymptomatic uterine perforations were encountered with the transcervical intrauterine catheter. Two of these were found incidently at cesarcan section and an additional suspected perforation was confirmed radiologically. There were no complications from these perforations. Scalp abscess and major fetal morbidity due to the application of the electrode were not seen in this study. There were 2 intrapartum stillbirths among the monitored labors.

Using a randomized double-blind research design, the efficacy and safety of intravenous prostaglandin F2a(PF) and oxytocin (O) for inducing labor were compared in term pregnant women. The results show that successful induction depends upon the condition of the cervix, and for comparable groups there was no difference for PF or O. The PF failures occurred in patients who were significantly more obese. The incidence of uterine hypertonus, hot flashes and vein phlebitis was increased significantly in the PF group but there was no difference in the incidence of fetal bradycardia, nausea, vomiting or diarrhea. The infants delivered vaginally were of similar weight and had similar 1- and 5-minute Apgar scores. Laboratory toxicity studies, before and after infusions (complete blood count, uric acid, bilirubin, SGOT, and blood urea nitrogen), were similar for the two drug groups, except that hemoglobin levels postinfusion were significantly higher in the O group. The incidence of cesarean section was significantly increased in the PF group and these sections were done for uterine hypertonus and/or fetal bradycardia. Caution should be exercised before prostaglandin Fα solutions are used routinely to induce labor at term.

Asymptomatic bacteriuria is thought to be associated with complications during late pregnancy or the puerperium. To understand this association, the underlying condition must be denned. This report describes findings in 53 women with asymptomatic bacteriuria early in pregnancy. Using a bladder washout tcchnic, the origin of bacteria was localized to the kidneys in 13 women (24.5%) and to the bladder in 19 (35.8%); in 7 (13.2%), the source was indeterminate. In the remaining 14 (26.4%), urine was spontaneously sterilized in an average period of 9.3 days after the screening test. Bacteriuria during pregnancy is obviously a heterogeneous entity, and it seems reasonable to anticipate that complications arise in only a particular subgroup of this population. This subgroup needs to be identified for rational prophylaxis and followup.

Numerous authors have correlated the deposition of creatinine in amniotic fluid with advancing gestation, although the mechanism of creatinine transport is less well defined. This study was undertaken, therefore, to elucidate the route of creatinine exchange in the maternal- fetal complex by injection of creatinine- 1-C14hydrochloridc into the maternal and fetal compartments of canines and subhuman primates in the final trimester of pregnancy. The results suggest that in dogs and monkeys creatininc traverses the placenta in both directions, and that the sequence, maternal serum &OV0142; fetal scrum &OV0142; fetal urine &OV0142; amniotic fluid, is physiologically constant as long as normal circulator)' dynamics are maintained. The finding of labeled creatinine in the amniotic fluid of ncphreclomized canine fetuses was viewed with considerable interest as an indication of an alternate route of creatinine transport in the dog.

Scrum levels of α2-macroglobulin, IgG, albumin and transferrin were estimated immunochemically in 45 pregnant women and 45 matched nonpregnant controls. There was a significant rise in α-macrogiobulin and transferrin but a decrease in IgG and albumin. Cryofiforinogenemia was detected in 389o of the samples from pregnant women, compared with 7% of the controls. The increase in u_-macroglobulin, possibly due to hormonal changes, may be one of the significant factors contributing to the increased incidence of thrombocmbolism during pregnancy. The frequency with which cryofibrinogenemia is detected in pregnant women points to the existence of subclinical in Ira vascular coagulation.

Thirty-six compound presentations of the fetus encountered over an 8-year-period are reviewed and analyzed with respect to incidence, obstetric complications, management and fetal outcome. Predisposing factors were prematurity, pelvic contracture, multiple gestation, increasing parity, and hydramnios. Management depends on the size of the fetus. Expectant management is the preferred treatment. Version and extraction, and reposition of the prolapsed extremity should be avoided. There seems no way to predict or prevent compound presentation, but once encountered a conservative approach is indicated.
