Abstract:This article reports on our experience with core needle biopsy (CNB) of the breast and presents a review of the literature. We have performed CNB on 304 women with abnormalities classified on imaging as highly suggestive of malignancy (52 cases), suspicious (245 cases) or probably benign (7 cases). The CNB procedure was performed using either stereotactic mammographic guidance (251 cases) or ultrasound guidance (53 cases) with a 14‐gauge needle. At least five specimens were obtained for each abnormality. The sensitivity of core biopsy was 98% and the specificity was 99%. There were three false negatives and one false positive. An excisional biopsy was recommended in 17 cases in which the CNB diagnosis was not definitive, including 12 with atypical ductal hyperplasia (ADH), 4 with discordance between imaging and histologic results, and 1 with inconclusive histology. Our results are consistent with other reports in the literature using 14‐gauge core needle biopsy. Abstract:Granuloma formation and chronic inflammation are local reactions that are associated with implantation of medical grade silicone. These responses lead to capsular contraction, pain, and cosmetic problems. In addition, there have been a few reports of connective tissue disease in patients with silicone gel‐filled implants. The purpose of this paper is to raise the possibility that local responses to silicone gel‐filled implants are mediated by fat necrosis.Results of histologic findings and patient records in six patients indicate that patients experiencing implant leakage or failure exhibited classic signs of a foreign‐body response, including the presence of giant cells and foamy macrophages. Most of the observed vacuoles were empty and appeared similar to enucleated fat cells, although some contained foreign material. These observations suggest that the physical presence or degradation products of the implants may have elicited fat cell necrosis that contributed to the chronic inflammatory response. In contrast, patients with intact implants showed no signs of a chronic foreign‐body response. None of the patients in this study demonstrated any systemic complications.It is concluded that although there have been numerous reports of empty vacuoles containing silicone that are either derived from the membrane that surrounds the implant or from the gel, there are no reports linking these vacuoles to the vacuoles seen in fat tissue necrosis, even though histologically the two processes are morphologically similar. Therefore, it is important to ask whether some of the adverse reactions observed with silicone gel‐filled implants may be mediated via fat cell

Abstract:Sensitive assays that recapitulate the complex and dynamic interaction of cells within the breast are needed to understand the early steps by which normal breast cells become cancerous, and cancer cells become invasive. We discuss the literature of available culture systems and elaborate on two assays we have developed to study interaction of normal and tumor cells with stromal cells and ECM in three dimensions. Abstract:Primary chemotherapy (the administration of chemotherapy prior to definitive locoregional therapy) of breast cancer has numerous potential advantages and several identified disadvantages. While primary chemotherapy has been used widely in the treatment of locally advanced and inflammatory breast cancers, interest is now turning to its use in resectable breast cancers. Within the last five years, several trials of primary chemotherapy in the treatment of resectable breast cancer, both randomized and non‐randomized, have been published. Although preliminary reports suggest that primary chemotherapy may be advantageous, the results from definitive randomized trials are not yet available. Abstract:This study examines the relationships of mood, age, living with a spouse, and time elapsed since diagnosis of first recurrence to sexual interest. Seventy‐two women with metastatic breast cancer and 26 of their spouses were recruited through their oncologists. Women and their spouses were administered a battery of measures designed to assess psychosocial and medical variables. Eighty‐three percent of the women and 100% of the spouses indicated that sexual pleasure was something they wanted to experience; however, only 25% of the women indicating this interest and only 19% of the spouses reported that it had been easy for them to experience sexual pleasure. For patients, living with a spouse was significantly associated with being interested in experiencing sexual pleasure [p<.05]. Patients with lower mood disturbance were significantly more able to experience sexual pleasure than those with high mood disturbance scores [p<.05]. Ability to experience sexual pleasure was significantly and positively correlated between patients and their spouses (r=.56, p<.01). Sexuality is an important aspect of quality of life for metastatic breast cancer patients and their spouses.
