摘要:
We discuss our experience with evaluation of 167 children by auditory brainstem response audiometry in 1978. We summarize experience with referral sources, medication for sedation, and interpretation of test outcome. Medication for sedation was required for 136 children. Concommitant central nervous system involvement rendered auditory brainstem response ambiquous in some children; however, the technique still provide useful information about status of peripheral auditory sensitivity obtainable in no other way. Finally, agreement among auditory brainstem response, behavioral, and impedance audiometry predictions was usually quite good.

摘要:
The purposes of this investigation were two-fold: (1) to determine whether differences between measured ear canal diameter and actual earplug size affect sound pressure levels in the ear canal during exposure to high levels of noise; and (2) to determine if individuals would select the insert earplugs that conform to their measured ear canal diameter if the decision were left to them. The results indicate that: (1) very small differences between measured ear canal diameter and actual earplug size affect considerably the sound pressure levels in the ear canal during exposure to high levels of noise; (2) individuals typically select earplugs that are smaller than their measured ear canal dimensions when the decision is left to them; (3) the most skilled technician cannot always determine ear canal sizes due to limitations of available measuring instruments.

Reflex decay to 0.5 kHz octave-band noise at 95 dB SPL was measured in chinchillas during four 2-hr exposure periods separated by 11 -min quiet intervals. Round window electrodes were implanted in six animals. Measures of acoustic reflex decay were inferred from amplitude changes in the cochlear microphonic generated by the octave-band noise. Reflex decay followed essentially the same time course during each exposure period. Vigorous muscular contraction to the signal onset was followed by gradual decay that a symptoted between 30 and 50% of its initial value. The process of decay seemed to be complete sometime between 8 and 30 min. The results suggest that the middle ear muscles in chinchillas provide some protective function during exposures of fairly long duration.

The Effect of Signal Type on the Loudness Discomfort Level |

Loudness discomfort levels (LDL's) were obtained on 19 normal-hearing subjects with 18 different stimuli using a simple up-down adaptive procedure. The stimuli included five pure tones, five one-third octave bands of noise, five one-third octave bands of filtered multi-talker babble, wideband noise, spondaic words, and sentences. Results indicated there was no significant difference among the three frequency-specific stimuli. However, a significant frequency effect was found. LDL's for the two speech stimuli and wideband noise were not significantly different and were similar to values for signals in the 500 to 4000 Hz region. Although differences among mean LDL's for the various signals were quite small, large intrasubject variability among the signal types and across frequency was observed. This fact would seem to preclude the conclusion that equivalent LDL's would be obtained on an individual regardless of the type of stimulus.

The use of the Staggered Spondaic Word Test (SSW) J. Katz et al., J. Speech Hear Disord. 33, 132–146, for measurement of central auditory ability in adults beyond the age of 60 years is presently limited by the lack of an adequate normative data base by which to differentiate typical and atypical response patterns obtained from elderly subjects. To provide a description of SSW performances for older age groups, the SSW was administered to 41 subjects ranging in age from 60 to 79 years. Average word discrimination scores for the experimental population were within normal limits. The subjects evidenced a decrease in response accuracy and an increase in response variability relative to previously reported SSW results obtained from younger subjects. Nonsignificant correlations between age and SSW scores suggest minimal change in those central auditory functions assessed by the SSW test for subjects between the ages of 60 and 79 years. The need for stricter control over cognitive awareness and capability in the design of experiments that involve elderly subjects is discussed.

Clinical Significance of the Relative Intelligibility of Pictorially Represented Spondee Words |

The relative intelligibility of 20 pictorially represented spondees were found for 40 normal hearing children between 3% and 5% years old. The range of the level of relative intelligibility was 5.3 dB. A spondee threshold (ST) was obtained for 60 different children between 3% and 5% years old in a 2 dB (N = 30) or a 5 dB step (N = 30) method for three sets of pictorially represented spondees (six pictures per set) having low, mid, and high levels of relative intelligibility. The ST's using a 2- or 5-dB step were not significantly different regardless of picture set level of relative intelligibility. ST's obtained with the picture sets having different levels of relative intelligibility were significantly different from each other. It was suggested that clinical consideration be given to the use of pictorially represented spondees which were identified at the lowest levels of relative intelligibility to increase the efficiency and precision of the ST measurement in children.

摘要:
PI-PB functions were defined and analyzed for 66 aged listeners (1 08 ears). Abnormal rollover b0.40) was identified in four subjects (four ears). The current study showed the frequency and magnitude of the rollover phenomenon to be greater in an aged population when compared to the general clinical population. The diagnostic and rehabilitative applications of the PI-PB function and the rollover phenomenon are discussed.
