摘要:

With the viral causation of murine mammary cancer firmly established, it appears most unlikely that the striking parallels found in the human disease can be coincidental. Thus in both species electron microscopy demonstrates typical virus-like particles in tumor tissue and milk, and immunologic studies indicate the presence of antibodies in the serum. It may be time to act on the clinical implications of these findings.

Although the majority of patients who develop dumping syndrome after partial gastrectomy can be treated successfully by medical means, some inevitably require a second operation. In the case presented, as in others, interposing a segment of jejunum in theantirather thanisoperistaltic relationship was highly effective in delaying the emptying of the gastric remnant and restoring its reservoir function.

In our civilization death is too often viewed as a failure of the health care system and the terminally ill patient seldom receives the emotional as well as physical support he needs in order to“die well.”But, as a pioneering unit has learned, when the hospital conceives its role to be the relief of suffering in all dimensions, an organized approach can bring positive benefits not only to the patient and his family but to the hospital staff as well.

By initiating and then sustaining the thrombotic process without involving the mechanisms of blood clotting, platelet aggregation plays a significant, if not yet fully defined, role in atherosclerosis and its complications. This finding suggests a new focus in therapy: the use of agents that inhibit platelet function (of which several are available) in preference to those that primarily affect coagulation.

In renal disease the persistence of antibiotics, or their serum half-life, depends on the extent to which the kidneys effect their clearance. For drugs largely or moderately cleared by the kidneys, serum creatimine concentration provides a method of gauging persistence and of adjusting dose schedules to the needs of the renally compromised individual. In urinary tract infections, drugs must be selected that reach the urine in adequate concentrations for efficacy.

The chorionic gonadotropin titer now makes it possible to identify early those postmolar patients who are likely to progress to choriocarcinoma, thereby opening the way to definitive treatment of this highly lethal condition. Complete and sustained remission can now be achieved in 90% of patients in whom appropriate chemotherapy is begun within four months of disease onset, and in 75% of all treated cases.
