摘要:
Using recent advances in microcomputer technology, a system for the continuous, direct processing of fetal monitoring information has been developed. The basic system consists of an Apple II microcomputer with accessories which are readily available at local computer stores. Eight fetal monitors are connected to the computer system by cable. Computer generated tracings, similar in quality to the original, are displayed on a monitor at the central unit and at the remote location in the labor and delivery area. This information also can be transmitted to physicians' homes and reproduced on a multi-copy graphic printer. Two benefits with this system were obtained. First, this application of microcomputer technology promotes the rapid dissemination of information to physicians and staff. Second, medical record storage can be improved by using the graphic printer's copies.

摘要:
A medical equipment preventive maintenance system using a Radio Shack microcomputer is described. The system generates a schedule of equipment to be inspected each week. The software is written in BASIC for easy modification or transfer to other commercially available microcomputers. The system has been in use for nine months with good results.

摘要:
The nontechnical aspects of medical computing systems are as important as the technical. In order to understand why, it is important to consider the social context of systems, the human engineering necessary for medical systems to work, and the specifics of the man-machine interface. It is also necessary to plan for the change which comes with any new method. This paper contains a discussion of these topics, concluding with a set of guidelines for change-management which we feel organizes the data of years of experience into a systematic approach.

A fully developed in-house clinical engineering program has much to offer to a large community hospital. In the southeast U.S., many community hospitals—600 beds or fewer—have implemented partial programs and, as a result, have received only a partial return on their investment. A comprehensive program extends maintenance support to all patient-care devices, is intimately involved in the new device specification, evaluation, and selection process with the medical staff—standardizing whenever appropriate—and is significantly involved in the continuing education program for in-house users of devices.

A significant change in the clinical engineering scene has occurred in recent years that has replaced qualitative and descriptive efforts by a quantitative biomedical science. In part, this change has been brought about by the advancement in the state-of-the-art of utilizing digital electronic computer systems calleddistributed data processing (DDP)systems. This paper discusses several commercially successful DDP systems presently available to the clinical engineer. A specialized form of distributed systems in which the processors communicate vialocal area network (LAN)is discussed. It promises an exciting opportunity for interconnecting complex electronic equipment in a large hospital.

The need for, and specific objectives of, a clinical internship for biomedical engineering students are discussed in Part I. Through such an internship, participants can gain an appreciation for the role that engineers can play in quantifying and automating medical procedures. They also learn about the organizational structure of a clinical facility, the regulations, safety standards and accreditation requirements imposed on the health care industry, and the technological needs as envisioned by medical personnel. Finally, students become familiar with information flow in a medical facility, the units, codes and standards used to report diagnostic information, and current technology utilized in health care delivery, especially its limitations.The administration of this internship program is discussed in Part II. The interns rotate through sequences of hospital services, divided into four sets, separated by intervening elective periods, and spread over different clinical facilities. Rotations include orientation conferences, counseling sessions with staff personnel, and on-the-job training in each hospital service. Students keep logs of their experiences and submit service reports summarizing each rotation. They also write brief reports illustrating engineering contributions to health care delivery. The reports are graded and serve, together with staff and faculty evaluations, to rate the students' overall performance in the program.

Computers are being used in increasing numbers to support health care facilities. Consequently, there is a need to examine how this technology can be effectively used to assist in the process of patient care. At the present time, most computer applications are directed toward (1)the management of large amounts of data during a short period of time(e.g., ECG analysis and physiological monitoring) or (2)the transfer of information within a facility(e.g., Hospital Information System (HIS) order entry/results reporting). Some systems use the data for the purposes of surveillance or patient management. Other systems are concerned with the organization and presentation of the clinical data as part of the decision making process. This paper considers the ways in which systems can effectively display information. It draws upon experience with theJohns HopkinsOncology Clinical Information System (OCIS).

The impact of selected image-processing schemes on diagnostic performance obtainable from computer-processed CT scans containing simulated nodular lesions was determined from controlled radiological observations. Normal CT scans of the upper abdomen were processed by a computer to simulate the presence of a randomly-located, disk-shaped, low-contrast, nodule within the liver stroma. Results for selected enhancement techniques were tallied using a balanced experimental design. The data indicate that suitably-processed displays significantly improve diagnostic performance expressed in terms of diagnostic errors, information transfer, and the implied noise-relative signal strength, d'. The same mode of display significantly reduces the time required in making diagnostic decisions relative to the status quo.
