摘要:
This paper constitutes a presentation to health care managers on the clinical engineering role in the hospital. Guidelines are covered that were issued in 1976, by the Joint Commission for the Accreditation of Hospitals: the engineering problems, goals, and solutions addressed in the clinical engineering function; the education of the clinical engineer; and the alternative methods of delivering engineering support services to the hospital. The material included in this paper can form the basis for presentations intended to increase awareness of the role of clinical engineering, improve compliance with JCAH requirements, and to stimulate action to make effective use of engineering.

Within the Greenville, South Carolina, Hospital System, an in-house Clinical Engineering Department is being developed as a shared service. This paper discusses the background for this program in a multihospital system. Several aspects of the program, including staffing, equipment needs, scheduling, participation in system activities, and some future possibilities for department expension are also presented.

摘要:
Periodic performance testing of biomedical equipment can be made considerably more efficient by careful design of test procedures and by fabrication of special test instruments which are designed for those procedures. The design philosophy behind such procedures and instruments and its applicability to a wide variety of biomedical devices is discussed. As a practical example, an ECG machine/patient monitor test system is described and construction details are given.

摘要:
The design, construction, and evaluation of an internally worn laryngeal prosthesis for laryngectomized patients is presented. Existing speech rehabilitation techniques were analyzed. The reed-fistula technique was adopted as having the most potential for effective design improvement. Its deficiencies were explored, and an alternative strategy for an internal prosthesis was formulated. Prosthetic mechanisms which met the design criteria were developed, and their performance was evaluated bothin vitroandin vivo.In this modified technique, the prosthesis is worm internally at the end of a surgically constructed fistula leading to the base of the patient's vocal tract. Exhaled air powers a miniature vibratory mechanism, whose output enters the vocal tract directly and supplies both a voiced and a voiceless sould source. Two patients using the new mechanism have regained a reasonable, usable speech with a minimum amount of effort and practice. An intelligibility test rated one patient's speech clearly superior over electronic and conventional reed-fistula speech.

This article reviews the literature and presents a number of hypotheses dealing with effects of electric currents on regeneration of body tissues. Data were reviewed in terms of potentials and/or potential gradients which were present in the experiments. The presence of an electrophoretic action which may be a valuable tool in preventing shock due to electrolyte imbalance in the treatment of burns was postulated.

摘要:
A series of in-vitro experiments conducted in our laboratory has shown that the echographic technique can be extended to quantify diaphragm motion in the total artificial heart (TAH). The data thus obtained have been utilized to compute a pump-characteristic-coefficient which incorporates the scale factor and surface area of the TAH pump diaphragm. This coefficient was found to be constant for all pumps of the same design. The product of the height of excursion of the diaphragm, as obtained by echocardiography, and the characteristic coefficient give the stroke volume of the pump and thus the cardiac output. With the availability of implantable echotransducers, it is possible to monitor the diaphragm motion. This technique has value in the non-invasive determination of stroke volume of total artificial hearts of the diaphragm type.
