AbstractOn August 1, 1985, an accident occurred at Triad Chemical damaging portions of the primary reformer. The accident was caused by ignition of natural gas pockets which entered the system during attempts to light an auxiliary boiler burner.The article reviews the cause of the accident, the resulting damage, the required repair, and the modifications made to prevent a recurrence.
